This report examines participation in the SBP among 3rd grade public school students, as well as the impacts of the program on food insecurity and children's risk of skipping breakfast. Students are more likely to participate when breakfast is served in the classroom, when time available for breakfast in school is longer, and when they come from lower income or time-constrained households. Abstract: Child health promotion is a salient public health goal. Childhood obesity rates have risen dramatically over the past few decades with more than 20% of youths overweight or obese by the time they enter middle school. Population-based strategies including those delivered through schools are needed to positively impact this trend. The IMOVE program evaluation, a quasi-experimental study involving one intervention and one comparison school, examines the influence of a school-based healthy eating program in a middle school cafeteria serving low-income, racially-diverse adolescents. Study 1 examined the association between the availability of healthy lunch meals (IMOVE meals) in school cafeterias and lunch and snack food purchase patterns of students. The introduction of IMOVE resulted in significantly less participation in purchase of low nutritional quality snack foods from fall to spring term. Students in both schools had similarly high participation in school lunch all year long, suggesting that IMOVE participation was sustainable. Study 2 examined the association between body mass index and purchase of IMOVE and standard school lunch meals in the intervention school. This study also identified sociodemographic predictors of participation in the school lunch program. Students who were overweight or obese had a significantly higher rate of purchase of both types of lunches, but even more so for standard school lunch than for IMOVE meals, compared to students who were not overweight/obese. Other characteristics significantly associated with participation in IMOVE and standard school lunch were nonwhite race, sixth grade, and low-income status. Study 3 examined the association between exposure to IMOVE and total daily food and nutrient intake measured using an abbreviated food screener before and after the intervention. At follow-up, students in the IMOVE school consumed sugary beverages and higher-fat milk offerings less often in their daily diets than students in the comparison school. Efforts to combat childhood obesity through changes in the school food environment have some impact, but require the support of policy action and nutrition education initiatives to be most effective. The IMOVE program proved feasible, acceptable, and sustainable, providing a model upon which to build additional intervention components to more comprehensively impact student wellness.

Abstract This study attempts to measure the linkages between participation in the School Breakfast Program and childrens nutritional status. The effects of the School Breakfast Program are examined using short term indicators including total fat, saturated fat, and cholesterol consumption and long term measures such as body mass index and likelihood of being overweight and at-risk for overweight. Analysis of data from the 2001-2002 National Health and Nutrition Examination Survey show that students participating in the School Breakfast Program consume significantly less total fat, saturated fat, and cholesterol than non-participants. While school breakfast participants have lower BMIs and a lower likelihood of being classified as overweight or at-risk for overweight than non-participants, these relationships were not statistically significant. While participation in the School Breakfast Program appears to result in a healthier diet, additional research is required to address the effects of the School Breakfast Program on childrens overweight status.

Creating Equal Opportunities for a Healthy Weight is the summary of a workshop convened by the Institute of Medicine's Standing Committee on Childhood Obesity Prevention in June 2013 to examine income, race, and ethnicity, and how these factors intersect with childhood obesity and its prevention. Registered participants, along with viewers of a simultaneous webcast of the workshop, heard a series of presentations by researchers, policy makers, advocates, and other stakeholders focused on health disparities associated with income, race, ethnicity, and other characteristics and on how these factors intersect with obesity and its prevention. The workshop featured invited presentations and discussions concerning physical activity, healthy food access, food marketing and messaging, and the roles of employers, health care professionals, and schools. The IOM 2012 report Accelerating Progress in Obesity Prevention acknowledged that a variety of characteristics linked historically to social exclusion or discrimination, including race, ethnicity, religion, socioeconomic status, gender, age, mental health, disability, sexual orientation or gender identity, geographic location, and immigrant status, can thereby affect opportunities for physical activity, healthy eating, health care, work, and education. In many parts of the United States, certain racial and ethnic groups and low-income individuals and families live, learn, work, and play in places that lack health-promoting resources such as parks, recreational facilities, high-quality grocery stores, and walkable streets. These same neighborhoods may have characteristics such as heavy traffic or other unsafe conditions that discourage people from walking or being physically active outdoors. The combination of unhealthy social and environmental risk factors, including limited access to healthy foods and opportunities for physical activity, can contribute to increased levels of chronic stress among community members, which have been linked to increased levels of sedentary activity and increased calorie consumption. Creating Equal Opportunities for a Healthy Weight focuses on the key obesity prevention goals and recommendations outlined in Accelerating Progress in Obesity Prevention through the lens of health equity. This report explores critical aspects of obesity prevention, while discussing potential future research, policy, and action that could lead to equity in opportunities to achieve a healthy weight.

Participation in the federally-subsidized school breakfast program often falls well below its lunchtime counterpart. To increase take-up, many districts have implemented Breakfast in the Classroom (BIC), offering breakfast directly to students at the start of the school day. Beyond increasing participation, advocates claim BIC improves academic performance, attendance, and engagement. Others caution BIC has deleterious effects on child weight. We use the implementation of BIC in NYC to estimate its impact on meals program participation, BMI, achievement, and attendance. While we find large effects on participation, our findings provide no evidence of hoped-for gains in academic performance, nor of feared increases in obesity. The policy case for BIC will depend upon reductions in hunger and food insecurity for disadvantaged children, or its longer-term effects. As the public health threat of childhood obesity has become clear, the issue has become the focus of local, state, and national initiatives. Many of these efforts are centered on the community environment in recognition of the role of environmental factors in individual behaviors related to food and physical activity. In many communities, for example, fresh produce is not available or affordable, streets and parks are not amenable to exercise, and policies and economic choices make fast food cheaper and more convenient than healthier alternatives. Community efforts to combat obesity vary in scope and scale; overall, however, they remain fragmented, and little is known about their effectiveness. At the local level, communities are struggling to determine which obesity prevention programs to initiate and how to evaluate their impact. In this context, the Institute of Medicine held two workshops to inform current work on obesity prevention in children through input from individuals who are actively engaged in community- and policy-based obesity prevention programs. Community perspectives were elicited on the challenges involved in undertaking policy and programmatic interventions aimed at preventing childhood obesity, and on approaches to program implementation and evaluation that have shown promise. Highlights of the workshop presentations and discussions are presented in this volume.
