Quelles difficultés ont-ils rencontrées ? PARTIE 1 : Méthodes : Analyse secondaire du fichier des thèses de médecine générale soutenues entre 2003 et 2008. Résultats : Sur 622 thèses, 20 % traitent des soins primaires, 13 % sont dirigées par un médecin généraliste. Discussion : Ces valeurs sont basses, notion historique s'expliquant partiellement par les difficultés rencontrées par les médecins généralistes pour diriger ces thèses. PARTIE 2 : Méthodes : - Enquête par questionnaire des 37 médecins généralistes directeurs d'au moins une thèse de soins primaires. - Entretiens semi-structurés auprès de 8 enseignants cliniciens ambulatoires sans expérience de direction. Résultats / - Les directeurs expriment le manque de compétences méthodologiques (58,3 % formés à la direction) et de temps (moyenne 28 h sur 12,7 mois/thèse). - Les enseignants cliniciens ambulatoires citent : manque de temps, de sollicitation par les étudiants, de formation, intérêt avant 45 ans et démotivation après 50 ans. DISCUSSION : L'investigation retrouve les difficultés techniques secondaires à l'insuffisance de la formation initiale et aux spécificités de l'exercice ambulatoire libéral. SYNTHESE-CONCLUSION : Seraient nécessaires la formation à la direction de thèse et à la recherche en soins primaires, une cellule de thèse pour aide méthodologique, proposition de sujets, amélioration du contact étudiants-enseignants cliniciens ambulatoires et surtout une action politique concordante avec les objectifs de santé publique. Loin de l"excellence", l'amélioration du nombre et de la qualité des thèses de médecine générale et leur publication témoigneraient de l'efficience de la filière.

Jean-Baptiste Say (1767–1832) was one of the first great economists to have laid down the foundations of economic science. Author of the famous Treatise on Political Economy in 1803, which was revised and re-edited on several occasions, he published numerous other works including a voluminous Complete Course in Practical Political Economy in 1828–9. He also taught political economy successively from 1815 until his death in three Parisian establishments: the Athénée, the Conservatory of Arts and Trades, and the Collège de France. The texts in which Say exposes his approach to political economy have not been available in the English language until now except for the fourth edition of the ‘Preliminary Discourse’ which serves as an introduction to the Treatise. This book presents a translation which renders his works accessible to the English speaking world. For the first time, English readers will be able to become directly immersed in Say’s principal texts, where he develops his conception of political economy. Jean-Baptiste Say and Political Economy proposes a translation of a selection of eleven of Say’s texts. The first three are versions of the ‘Preliminary Discourse’ from the Treatise’s editions of 1803, 1814 and 1826 with the variations of the editions of 1817, 1819 and 1841. The following four texts are the opening discourses pronounced at the Conservatory in 1820 and 1828 and the Collège de France in 1831 and 1832. The eighth text is the ‘General Considerations’ which open the Complete Course in Practical Political Economy of 1828, with the variations of the 1840 re-edition. The final three texts are those Say devotes to ‘the progress of political economy’ in what is akin to a history of economic thought. This volume is of great importance to economic historians and people studying Jean-Baptiste Say, as well as those who are interested in economic theory and philosophy and political economy.

In the late 1950s, like tens of thousands of young men of his generation, Pierre Bourdieu, having recently passed the agrégation in philosophy, found himself immersed in the Algerian war. Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. These new tools also enabled him to analyse the nature of the crisis that the war had both produced and manifested. Much orthodox economic theory is based on assumptions which are treated as self-evident: supply and demand are regarded as independent entities, the individual is assumed to be a rational agent who knows his interests and how to make decisions corresponding to them, and so on. But one has only to examine an economic transaction closely, as Pierre Bourdieu does here for the buying and selling of houses, to see that these abstract assumptions cannot explain what happens in reality. As Bourdieu shows, the market is constructed by the state, which can decide, for example, whether to promote private housing or collective provision. And the individuals involved in the transaction are immersed in symbolic constructions which constitute, in a strong sense, the value of houses, neighbourhoods and towns. The abstract and illusory nature of the assumptions of orthodox economic theory has been criticised by some economists, but Bourdieu argues that we must go further. Supply, demand, the market and even the buyer and seller are products of a process of social construction, and so-called ‘economic' processes can be adequately described only by calling on sociological methods. Instead of seeing the two disciplines in antagonistic terms, it is time to recognize that sociology and economics are in fact part of a single discipline, the object of which is the analysis of social facts, of which economic transactions are in the end merely one aspect. This brilliant study by the most original sociologist of post-war France will be essential reading for students and scholars of sociology, economics, anthropology and related disciplines.
