Lorsque la maladie débute par un épisode maniaque ou une hypomanie évidente et repérée, le diagnostic ne pose en général pas de problème. En revanche lorsque la maladie débute par un épisode dépressif majeur, la question est de savoir s'il s'intègre dans un trouble unipolaire ou dans un trouble bipolaire. Le rôle du médecin généraliste est d'évoquer la possibilité de trouble bipolaire face à un syndrôme dépressif récurrent et, le cas échéant, d'instaurer le traitement. L'objectif de la thèse est double : analyser en profondeur le point de vue des médecins généralistes, interlocuteurs de choix pour dépister et orienter les patients, puis élaborer avec eux un outil de dépistage de l'hypomanie en soins primaires dans le cadre de syndrôme dépressif récurrent. Afin d'encourager les médecins à s'exprimer, la méthode d'investigation la plus adéquate est qualitative : elle consiste à mener un groupe de discussion, avec l'aide d'un modérateur. L'analyse de la discussion montre que leur priorité n'est pas tant de poser le diagnostic de bipolarité, mais de minimiser le risque suicidaire. Ils montrent une réelle réticence à suivre un questionnaire-type, long et formel, face à un patient dépressif. Le questionnaire élaboré au cours de la réunion est donc court, les questions faciles à poser lors d'une première consultation. Cet outil peut être utilisé en première intention, il semble plus appropriée aux yeux des médecins participants et contribuerait à réduire les délais de diagnostic.

Le trouble bipolaire est une pathologie fréquente touchant entre 1 et 3 % de la population mondiale. Ce trouble est à l’origine d’un retentissement important sur la sphère personnelle, familiale et socio-professionnelle avec un risque suicidaire accru. Le dépistage de la dépression bipolaire est difficile en raison de la multiplicité des tableaux cliniques et des limites des classifications actuelles. Pourtant, ce dépistage permet d'améliorer notablement la prise en charge du trouble. Notre revue de littérature est réalisée à partir d’une recherche MEDLINE sur une période allant de 1981 à Juin 2009 en utilisant les termes principaux : « Bipolar disorder », « Depression », « Depressive disorder », et « Primary health care ». Les résultats de notre travail ont permis de distinguer des facteurs prédictifs de bipolarité chez des patients déprimés. Afin de systématiser son entretien clinique, le médecin généraliste peut s'appuyer sur quatre questionnaires standardisés : le SAD-P, le MDQ, la CLH-32 et la MAThyS. Cependant, ils présentent plusieurs limites, notamment un aspect chronophage en médecine générale, ainsi qu'une spécificité moyenne pour les troubles bipolaires de type II. Chez un patient déprimé, le diagnostic de bipolarité ne pourra donc pas être retenu par un nombre restreint d’indices, il repose sur un faisceau d’arguments au terme d’un entretien clinique systématisé. Le dépistage d'un tel trouble chez un patient déprimé représente un enjeu considérable en ce qui concerne la prise en charge thérapeutique. D'après les conférences de consensus, les thymorégulateurs sont le traitement de première ligne. Si le praticien décide de prescrire un antidépresseur, une période courte est recommandée, sous couvert d'un thymorégulateur. L'amélioration de la prise en charge de la dépression bipolaire passe donc par une optimisation de la formation des professionnels de santé et par une meilleure collaboration entre les médecins généralistes et les psychiatres. Dans cette optique, le développement des réseaux de soins et des centres référents peut se révéler une aide précieuse pour le médecin généraliste.

Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments?
