Les doléances évoquées par les patients totalement édentés porteurs de prothèses amovibles complètes (PAC) maxillo-mandibulaires compliquent les rapports de confiance entre patient et praticien, indispensables à l'intégration fonctionnelle et psychologique de nos traitements. Afin de limiter les situations conflictuelles, des recommandations de bonnes pratiques ont été énoncées par les autorités de santé. La loi du 4 mars 2002 rappelle également le droit des malades et les obligations des professionnels de santé notamment en termes d'informations, de moyens et de résultats. Le consensus décidé en mai 2002, à l'université McGill de Montréal préconise l'adjonction de deux implants symphysaires connectés à une base prothétique complète comme minimum thérapeutique à l'arcade mandibulaire. Un exercice professionnel conforme aux données actuelles de la science se doit de profiter pleinement des possibilités thérapeutiques proposées par l'implantologie. On peut dès lors s'interroger sur les pratiques des chirurgiens-dentistes, sur le fait de proposer le replacement des dents par une PAC quand la prothèse implanto-retenue est considérée comme le standard minimum au niveau international. L'objectif de cette thèse était donc d'évaluer l'attitude thérapeutique des praticiens dans la prise en charge d'un patient totalement édenté. Nous avons réalisé une étude transversale en interrogeant un groupe de praticiens installés en Ile de France sur ses pratiques professionnelles. Il en ressort que si la majorité des praticiens a recours aux implants en première intention à l'arcade mandibulaire, près d'un tiers des praticiens propose une PAC. Cette attitude concerne essentiellement des praticiens de plus de 60 ans et sans diplôme relatif à l'implantologie.

L'édentement est un phénomène qui a toujours existé et auquel les chirurgiens-dentistes ont toujours été confrontés. Les solutions prothétiques ont été largement décrites dans la littérature que ce soit pour les édentements partiels ou totaux. L'édentement total, dans l'approche amovible est pris en charge par des prothèses totales reposant uniquement sur un support muqueux. Cependant l'édentement subtotal est une situation qui requiert une approche particulière. En effet, lorsque les quelques dents restantes sont encore conservables notamment sur l'arcade mandibulaire, elles peuvent constituer complément important de sustentation, de stabilisation et de rétention pour une prothèse amovible. Cette approche qui est psychologiquement plus acceptable pour certains patients permet donc d'obtenir un plus grand confort prothétique ainsi que d'appareiller des patients qui refuseraient qu'on « provoque » leur édentement complet. Il revient donc au chirurgien-dentiste, en intégrant des paramètres liés au patient, à l'arcade concernée, aux dents concernées et à ses compétences de proposer cette alternative lorsqu'il considère qu'elle a un intérêt pour son patient.
