Les carcinomes lobulaires invasifs sont un challenge pour le radiologue. En effet, leur diagnostic par l'imagerie standard apparaît plus difficile que celui des autres cancers. Notre étude de 39 cas montre l'interêt certain de l'IRM dans la prise en charge de ces patientes en complément de l'imagerie conventionnelle. Elle estime en effet au plus près la dimension exacte de la tumeur, donnée fondamentale pour un éventuel traitement conservateur. Elle permet également de manière bien plus sensible que toutes les autres techniques de rechercher une multifocalité/centricité, cause de récidives en cas de méconnaissance. Son principal écueil est son manque de spécificité qui oblige à obtenir une vérification histologique de tous les réhaussements détectés. La conséquence est une modification de l'attitude thérapeutique initiale dans plus de 50% des cas, le plus souvent au détriment d'une geste conservateur mais au bénéfice d'un traitement carcinologique complet, source d'un taux de récidive diminué.
