Although drinking, smoking and obesity have attracted social and moral condemnation to varying degrees for more than two hundred years, over the past few decades they have come under intense attack from the field of public health as an 'unholy trinity' of lifestyle behaviours with apparently devastating medical, social and economic consequences. Indeed, we appear to be in the midst of an important historical moment in which policies and practices that would have been unthinkable a decade ago (e.g., outdoor smoking bans, incarcerating pregnant women for drinking alcohol, and prohibiting restaurants from serving food to fat people), have become acceptable responses to the 'risks' that alcohol, tobacco and obesity are perceived to pose. Hailing from Canada, Australia, the United Kingdom and the USA, and drawing on examples from all four countries, contributors interrogate the ways in which alcohol, tobacco and fat have come to be constructed as 'problems' requiring intervention and expose the social, cultural and political roots of the current public health obsession with lifestyle. No prior collection has set out to provide an in-depth examination of alcohol, tobacco and obesity through the comparative approach taken in this volume. This book therefore represents an invaluable and timely contribution to critical studies of public health, health inequities, health policy, and the sociology of risk more broadly.

The need to reduce disability and premature deaths from non-communicable diseases (NCDs) is increasingly engaging international organisations and national and sub-national governments. Since the last meeting of the WHO Expert Committee on Problems Related to Alcohol Consumption WHO has undertaken a range of major initiatives to support Member States and reinforce the evidence on which policies work to develop global and regional information systems and to promote effective policies in health-care settings. These initiatives provide the background for the continuing role of WHO in supporting Member States to reduce the harm done by alcohol. In resolution WHA58.26 the Fifty-eighth World Health Assembly in 2005 requested the Director-General to report to the Sixtieth World Health Assembly on evidence-based strategies and interventions to reduce alcohol-related harm including a comprehensive assessment of public-health problems caused by harmful use of alcohol and to draw up recommendations for effective policies and interventions to reduce alcohol-related harm . To this end a WHO Expert Committee on Problems Related to Alcohol Consumption was convened with the main objectives of reviewing a range of public health problems attributable to alcohol consumption as well as scientific and empirical evidence of effectiveness of different policy options and providing technical recommendations on effective policies and interventions to reduce alcohol-related harm.

A working group of sixteen experts from seven countries re-evaluated the evidence of the carcinogenicity of betel-quid and areca-nut chewing and some areca-nut related nitrosamines. Betel-quid and areca-nut chewing are widely practised in many parts of Asia and in Asian-migrant communities elsewhere in the world. There are hundreds of millions of users worldwide. They evaluated betel quid with tobacco as carcinogenic to humans (Group 1) on the basis of sufficient evidence of an increased risk of cancer of the oral cavity, pharynx and oesophagus. The working group reviewed epidemiological studies of human cancer, mainly studies from India, Pakistan and Taiwan (China). Studies on betel quid with tobacco and areca nut with tobacco in experimental animals now also provide sufficient evidence of carcinogenicity. The working group also evaluated betel quid without tobacco as carcinogenic to humans (Group 1), on the basis of sufficient evidence of an increased risk of oral cancer. Fighting the obesity epidemic has so far proven a difficult challenge, given the diversity of natural and processed foods, the complexity of food supply chains, and the fact that targeting excessive caloric consumption is far trickier than reducing overall consumption (as for tobacco). Nevertheless, efforts to curb caloric intake are gearing up and the experience from tobacco control has drawn much attention on a potential role for excise taxes in fighting obesity. Many related questions have therefore been raised as part of the IMF’s capacity development work: Should excises on unhealthy food be used to fight obesity? If so, under what conditions? What are the product and market characteristics that would help identify the relevant tax bases and the rates at which to tax them?
