In the decade since the first edition of this work was published, an incredible array of reproductive technologies and associated issues has emerged. Obstetricians and gynecologists are hard-pressed to deal with the startling breadth and depth of these issues, which require mastery over a daunting combination of ever-increasing scientific knowledge, technical skills, long hours, legal liability, and exposure to clinical situations of overwhelming emotional intensity. In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health.
