At the onset of many affections it is either general or is referred to the umbilical region, but later the situation Of the pain as referred to the abdominal wall is a very fair indication Of the position within the abdominal cavity of the seat of mischief. I would instance as examples the pain in the left upper quadrant of the abdomen in perforation of a gastric ulcer, the pain over the gall-bladder region in cholecystitis, in the loin and along the course Of the ureter in renal colic, in the epigastrium in pancreatic hemorrhage and acute pancreatitis, in the, right iliac fossa in appendicitis. In perforation of a typhoid ulcer the pain is frequently experienced at a point in the abdominal wall exactly overlying the site Of perforation. Excerpt from West London Medical Journal, 1898, Vol. 3 Perhaps too little has been done in ascertaining the part inheritance or environment play in determining the varying degrees of susceptibility to infectious diseases, cancer, and gout. Take the latter. In few things has there been a more satisfactory reformation than in dining and the subsequent drinking which used to follow it. No wonder that gout was thought hereditary in every family of consequence. Beginning early, men ate to satiety and Soaked afterwards like so many sand-beds. Tables groaning under ponderous joints mangled by the guests were the survivals of the barbaric banquets of Northmen and Saxons. The haemorrhagic diathesis affords much scope for future work, for doubtless we have here to deal with a structural defect in the blood-vessels rather than in the blood itself. It is well known that the children of the affected individual Often escape. All the Children of the sons, and the female children Of the daughters also escape, but the sons of the daughters are commonly affected. This fact was shown by a genealogical tree which I assisted in constructing some time ago in respect of cases of bleeders. Excerpt from The Eclectic Medical Journal, Vol. 57: January to December, 1897 I claim nothing new for the use of morphia in these cases except as to the time of its administration. The young doctor will make mis takes here as well as elsewhere. Not too early nor too late; just the happy mean, and on his ability to judge of that time, will depend his success. Don't be too impetuous in the management of your first labors. Make haste slowly. Study your cases carefully. No description is like a bedside experience in teaching you to see with your finger tips. I agree with the editor that a reasonably rapid labor is prob ably the safest; but too much haste is just as bad. The soft parts must have time to adjust themselves. The perineal muscles must be tired out, hence a too rapid advance, by natural or artificial means, may be equally destructive. I have told you that the explosive forms of labor did not give the peritoneal structure time for adjustment; you know that a muscle in a state of contraction is more easily torn than when relaxed. These explosive pains drive the head at the perineum in such an impetuous manner that they cause a sort of counter or sympathetic contraction of that structure, and before it can relax, the head may be forced through, and an irreparable damage done. Morphia in these cases may be of use in other directions. For example: I have seen puerperal convulsions occur just at the moment of the delivery of the head. Its use would be our best safeguard in such cases. I pursue this course in all cases of first labors, and for this reason you can not tell just when the labor will take. On this spasmodic con dition. I have had the most favorable looking cases end with one of these outbreaks - too late for treatment - and with laceration, of course. If, when called, you find that the 08 will barely allow the introduction of the tip of the finger, although the pains come every two or three minutes with force enough to make her black in the face, just give her hypodermically one-fourth of a grain of morphia. Stay long enough to see that it produces no unpleasant effects, then go home and wait till they call you.
