Excerpt from Transactions of the Section on Preventive Medicine and Public Health The colonial period brought forth an attempt on the part of the various ports and cities to exclude and eradicate pestilential disease. This was a local, nu organized, and non-uniform movement, too often influenced by commercial motives and civic pride, but the seed of preventive medicine thus sown took root, and one of the first bills introduced into the First Congress of the United States provided for the assump tion of the quarantine power by the federal govern ment. But the time was not ripe, and we passed through a long period of state and local quarantine, and not a few devastating epidemics, before the citizen ship had evolved to the point where it would impose these duties on the general government. Even then we were unprepared for the step, and the short-lived national Board of Health came and went, but not before it had touched with prophetic finger the heart of the problem of the public health, the state and local boards of health. In a republic where the right to govern rests on the consent of the governed, where public senti ment very largely influences the enforcement of sanitary law, where people think before they obey, the comer stone of the whole public health fabric is the state board of health, and its efficiency determines in large measure the efficiency of the whole. Coincident with the development of the state boards came the improvement of the federal health agencies. The national Board of Health was a de new creation springing fully formed from the brains of a number of the most brilliant and public-spirited medical men of their time. Excerpt from Transactions of the Section on Preventive Medicine and Public Health of the American Medical Association at the Sixty-Sixth Annual Session, Held at San Francisco, June 22 to 25, 1915 I should also like to bring out the importance of making practitioners in subtropical countries more acquainted with this and all other plague-like diseases, because it has twice happened that my assistants have made the diagnosis of plague, and were ready to bring about measures which would have been sure to quarantine Honduras, and I was able to see the cases before these steps had been taken. By making the proper examinations we were able to demonstrate that the cases were not plague. The serious results of such a mistake would be greater in the case of the Central American coun tries than in the United States, because the health authorities cannot go so far with the people there as here; and after one such error it is very likely that if true plague were later found, the necessary quarantine measures would be very much delayed. The mere exercise of police powers will not control disease. The people need advice and education. The masses are at least one generation behind the modern health officer, so far as their knowledge of preventive medicine is con cerned, and the health officer himself is even farther away from a complete knowledge of the subject. He therefore must be an investigator always striving after new truths. The days of the part-time, unskilled health officer and the old-time sanitary inspector are numbered. It is now realized that health officials must be thoroughly familiar with their work both in theory and in prae tice. With increasing knowledge among the people, popular opinion is delegating more and more authority to the health department and increasing its duties many fold.
