Le devinir et l'expression fonctionnelle de la sténose résiduelle après thrombolyse intracoronnarienne pour infarctus du myocarde ont été étudiés sur un groupe de 148 patients. La phase aiguë de l'infarctus est dominée par la survenue d'une ŕéocclusion dans plus de 20 % des cas. Ce risque est très favorablement influencé par une héparinothérapie efficace entreprise dès la fin de la procédure de thrombolyse. Celle-ci ramène le taux de réocclusion à des chiffres voisins de ceux notés dans la littérature lorsqu'une angioplastie transluminale a été d'emblée associée à la thrombolyse. En conséquence, hormis certains cas particuliers, la place de l'angioplastie et du pontage aorto-coronarien en phase aiguë d'infarctus est réduite aux cas d'ischémie mal contrôlée par le traitement médical. Au-delà de la phase aiguë, le risque de réocclusion est très faible, contrairement aux diverses manifestations d'ischémie résiduelle, notamment l'angor, lequel touche au moins le quart des patients. Ceux-ci bénéficient alors pleinement d'une angioplastie transluminale ou d'une chirurgie de pontage dont l'indication est portée à l'issue d'un bilan évaluatif pratiqué après plusieurs semaines de recul.

La thrombolyse par perfusion intracoronarienne de streptokinase à la phase aigue de l'infarctus a été proposée afin d'obtenir une revascularisation précoce de la zone ischémiée et ainsi de réduire l'étendue de la nécrose tissulaire. Le devenir à long terme de 99 patients traités selon cette méthode a été étudié en fonction du résultat (succès ou échec) de la tentative de désobstruction coronarienne. Dans le groupe des succès de la thrombolyse, la mortalité hospitalière est moindre , le développemet d'une insuffisance cardiaque au décours de l'accident coronarien est moins fréquent; par ailleurs, la réinsertion socio-professionnelle et la tolérance psychologique de la maladie sont meilleures. Par contre, la mortalité à distance de l'infarctus ne diffère pas dans les deux groupes. Il en est de même en ce qui concerne la fréquence des récidives de nécrose myocardique, de l'angor résiduel et des troubles du rythme. Cette technique devant être mise en oeuvre dans des délais brefs et nécessitant une infrastructure lourde, garde une application restreinte en dépit de ses résultats intéressants. La nouvelle génértaion d'agents thrombolytiques devait apporter des solutions à ces problèmes par leur commodité d'utilisation.
