Le syndrome d'apnées obstructives du sommeil (SAOS) est hautement prévalent dans la population générale. Du fait d'une couverture diagnostique générale encore insuffisante, près de la moitié des patients dont le SAOS nécessiterait un traitement ignorent leur pathologie. Il existe possiblement une morbidité associée au SAOS, liée à l'absence de diagnostic et/ou de traitement de ces patients. Le but de cette revue est de faire le point sur les données épidémiologiques et l'impact péri-opératoire du SAOS, depuis la consultation d'anesthésie (dépistage), jusqu'à la gestion per-opératoire. Existe-t-il un fondement suffisant pour statuer si le SAOS est lié à des complications postopératoires et nécessite une gestion de l'antalgie et de la ventilation spécifique ? Les évènements respiratoires sont les principales complications observées en post-opératoire. La physiopathologie de l'aggravation du SAOS reste encore peu définie, et l'absence d'essai interventionnel disponible suffisant ne permet pas d'affirmer qu'un traitement spécifique est requis pour ces patients. La consultation d'anesthésie est, en plus d'être un moment pour dépister une pathologie cardio-vasculaire, une occasion de suspecter le SAOS. Ceci est important car la simple suspicion de cette pathologie permet d'anticiper les éventuelles difficultés de la prise en charge péri-opératoire. Nous proposons une étude (Anesthésie, Arythmies, Apnées : 3A) qui a débuté au CHU de Grenoble et permettra une approche physiopathologique par un dépistage du SAOS après la consultation anesthésie et l'analyse des anomalies électrocardiographiques dans un contexte de routine clinique post-opératoire.
