Found in young persons, more frequent in the young physically and mentally uh stable woman, unhappily married. Genuine angina pectoris appears at a later age (45 to is in most cases independent Of psych genic factors and depends strongly upon physical exertion. Between the attacks the patients do not impress one as being ill or nervous. In the nervous forms, no pains are experienced at night and no attacks occur. Of great significance in the differential diagnosis are previous diseases. In these cases' the heart and vessels are usually normal. In true angina pectoris the vessels of the heart show organic changes by the ecg. Tracings. In the functional type no path ologic changes can be demonstrated. The accounts of the patient in the nervous forms are usually dramatic: I was sure death had struck me, or I was sure I was dying. These cases as a rule have a rapid heart rate. In organic diseases the pain is as of a dagger piercing the chest or Of a claw gripping the heart hard; with radiation of the pain to the left shoulder and arm, rarely to the right shoulder and arm and into the neck and back, or to either or both angles of the jaw. In a very short time the patient can perform considerable work. In contrast, a heart after a real attack of angina is not capable of carrying on its work in a normal way.
