Book Description
This scoping review aimed to identify alternative purchasing arrangements (apart from fee for service [FFS] and pay for performance [P4P]) and their associated service delivery models that countries have used to improve the quality of chronic care, as well as to assess how these purchasing arrangements have impacted the quality of care. A scoping review of academic and grey literature published between 2013 and 2022 was carried out on alternative purchasing arrangements and their impact on the quality of chronic care using causal inference methods. The analytical approach consisted of clustering articles around similar purchasing arrangements or programmes, summarizing the evidence across the clusters, and highlighting the service delivery aspects and institutional design features that potentially impacted the quality of care. This research provided information to the WHO and OECD joint publication Purchasing for quality chronic care: summary report.

Book Description
There has been significant interest globally in understanding how purchasing arrangements can be designed strategically to optimize the delivery of health services. Given the growing burden of chronic diseases worldwide and the costs they are likely to impose on health systems, it is imperative that health services for chronic diseases are purchased in a way that contributes to the objectives of universal health coverage, including improved quality, and ultimately achieves better health outcomes. This paper explores the role of purchasing arrangements in improving the quality of chronic disease care. Specifically, the paper provides an overview and summary of the types of payment methods that have been used, summarizes evidence of their effectiveness in improving the quality of chronic care and assesses the enablers and barriers to the implementation of purchasing reforms. For that matter, a scoping review of the peer-reviewed literature was conducted, focusing on payment methods that seek to improve the quality of chronic disease care published. The lessons from these payment initiatives implemented to improve the quality of chronic disease care, mainly from HICs, suggest that they are a promising tool but, nonetheless, the quality and equity aspects need more attention in terms of the design of such initiatives as well as in terms of evidence generation. The significant information technology infrastructure and resources required to implement such arrangements potentially limit their applicability in low-resource settings.

Book Description
The private sector’s role in healthcare is growing across many settings. However, the sector remains under-governed in many contexts, particularly in low- and middle-income countries. Furthermore, the understanding of the evidence-base relating to private sector governance remains inadequate, with limited information available on the effectiveness of various approaches, and factors which facilitate or hinder their functioning. This scoping review was commissioned by the World Health Organization (WHO) to address this gap by synthesizing the available literature on the governance of mixed health systems.

Book Description
Historically, regulations governing chemical use have often focused on widely used chemicals and acute human health effects of exposure to them, as well as their potential to cause cancer and other adverse health effects. As scientific knowledge has expanded there has been an increased awareness of the mechanisms through which chemicals may exert harmful effects on human health, as well as their effects on other species and ecosystems. Identification of high-priority chemicals and other chemicals of concern has prompted a growing number of state and local governments, as well as major companies, to take steps beyond existing hazardous chemical federal legislation. Interest in approaches and policies that ensure that any new substances substituted for chemicals of concern are assessed as carefully and thoroughly as possible has also burgeoned. The overarching goal of these approaches is to avoid regrettable substitutions, which occur when a toxic chemical is replaced by another chemical that later proved unsuitable because of persistence, bioaccumulation, toxicity, or other concerns. Chemical alternative assessments are tools designed to facilitate consideration of these factors to assist stakeholders in identifying chemicals that may have the greatest likelihood of harm to human and ecological health, and to provide guidance on how the industry may develop and adopt safer alternatives. A Framework to Guide Selection of Chemical Alternatives develops and demonstrates a decision framework for evaluating potentially safer substitute chemicals as primarily determined by human health and ecological risks. This new framework is informed by previous efforts by regulatory agencies, academic institutions, and others to develop alternative assessment frameworks that could be operationalized. In addition to hazard assessments, the framework incorporates steps for life-cycle thinking - which considers possible impacts of a chemical at all stages including production, use, and disposal - as well as steps for performance and economic assessments. The report also highlights how modern information sources such as computational modeling can supplement traditional toxicology data in the assessment process. This new framework allows the evaluation of the full range of benefits and shortcomings of substitutes, and examination of tradeoffs between these risks and factors such as product functionality, product efficacy, process safety, and resource use. Through case studies, this report demonstrates how different users in contrasting decision contexts with diverse priorities can apply the framework. This report will be an essential resource to the chemical industry, environmentalists, ecologists, and state and local governments.

Book Description
The present text is the revised/updated version of the CIOMS International Ethical Guidelines for Biomedical Research Involving Human Subjects. It consists of 21 guidelines with commentaries. A prefatory section outlines the historical background and the revision process and includes an introduction an account of earlier instruments and guidelines a statement of ethical principles and a preamble. An Appendix lists the items to be included in the research protocol to be submitted for scientific and ethical review and clearance. The Guidelines relate mainly to ethical justification and scientific validity of research; ethical review; informed consent; vulnerability - of individuals groups communities and populations; women as research subjects; equity regarding burdens and benefits; choice of control in clinical trials; confidentiality; compensation for injury; strengthening of national or local capacity for ethical review; and obligations of sponsors to provide health-care services. They are designed to be of use to countries in defining national policies on the ethics of biomedical research involving human subjects applying ethical standards in local circumstances and establishing or improving ethical review mechanisms. A particular aim is to reflect the conditions and the needs of low-resource countries and the implications for multinational or transnational research in which they may be partners.
