Résumé :Introduction : Le dépistage de la consommation de cannabis chez l'adulte est sous-réalisé en médecine primaire. Seuls 8% des médecins généralistes le pratiquent systématiquement avec leurs patients, alors que le cannabis est le produit illicite le plus consommé en France. L'objectif de cette étude était de comprendre les raisons précises pour lesquelles les médecins n'abordaient pas cette problématique, en les interrogeant sur leurs pratiques et leurs ressentis. Matériel et méthode : étude qualitative par entretiens individuels semi-dirigés réalisée auprès de 13 médecins généralistes Franc-Comtois.Résultats : Les médecins ne pratiquaient pas le dépistage systématique et très peu le dépistage opportuniste. Le dépistage indispensable auprès des patients présentant des symptômes était plus fréquent, et ils abordaient peu la question en dehors du contexte évident. Les freins avancés étaient la peur de la réaction du patient, le manque de temps, le manque d'intérêt pour le dépistage. le manque de maîtrise du sujet, et la difficulté à identifier les consommateurs. Les outils pour le dépistage et l'intervention auprès des consommateurs étaient peu connus, même si les médecins identifiaient bien leur rôle de prévention dans la délivrance de l'information minimale. S'ils étaient capables d'adresser leurs patients de façon satisfaisante, ils avaient du mal à intervenir par eux-même sur des consommations moins problématiques.Conclusion : La majeure partie des freins au dépistage pourrait être levée par une meilleure sensibilisation au sujet et par l'apprentissage de l'utilisation d'outils simples et disponibles concernant le dépistage et la prise en charge des usagers de cannabis.

Introduction : Partant de situations pratiques vécues en consultation, pendant lesquelles il fut difficile de répondre aux attentes de patients demandeurs de conseils à propos du Cannabidiol (CBD), je me suis interrogé sur l'utilisation du CBD en France et sur les connaissances et le rôle des médecins généralistes de Bourgogne Franche-Comté (BFC) sur le sujet. Le marché lié à cette molécule est en expansion constante depuis son apparition en France en 2018, sans que la législation réponde entièrement aux enjeux de ce développement. De ce fait une confusion entre cannabis à usage récréatif, cannabis thérapeutique, et CBD s'entretient, notamment sur le plan législatif. La consommation du CBD en vente libre se fait majoritairement pour des raisons de bien-être et de santé, sans que ces produits soient considérés comme des médicaments. Les médecins généralistes pourraient donc représenter une personne ressource pour les utilisateurs à la recherche de conseils, mais ont-ils les connaissances nécessaires pour jouer ce rôle ? Méthode : La première partie est le fruit d'une revue de la littérature sur le CBD, illustrée par les résultats d'entretiens exploratoires. Les entretiens ont été menés avec des acteurs en rapport avec le cannabidiol, à savoir deux commerçants d'une boutique de CBD et un médecin algologue. La seconde partie présente les résultats des réponses des médecins généralistes de BFC à un questionnaire interrogeant leurs pratiques médicales vis-à-vis du CBD. Résultats : Les connaissances des médecins généralistes de BFC à propos du CBD sont limitées, manquant de précision, notamment sur le plan pharmacologique. La majorité des médecins (75%) a déjà été sollicité par leurs patients à propos du CBD. Seulement 16% des médecins interrogés déclarent se sentir capable de conseiller leur patient. Les facteurs qui semblent avoir une influence sur le rôle des médecins sont les suivants : le lieu d'exercice, le statut de médecin installé, la connaissance de différentes formes de CBD et de point de vente du produit, l'usage de CBD : personnel ou de la part d'un proche, le fait de déclarer avoir des connaissances théoriques sur le cannabidiol. Conclusion : Les médecins généralistes de BFC ont des connaissances limitées sur le CBD en tant que molécule. Les connaissances théoriques sont succinctes et peu précises et notamment sur l'aspect pharmacologique. Néanmoins les médecins auraient un rôle à jouer assez important étant donné qu'une grande majorité a déjà été sollicité par des patients pour avoir des conseils quant à l'usage du produit.

Central banks came out of the Great Recession with increased power and responsibilities. Indeed, central banks are often now seen as 'the only game in town', and a place to put innumerable problems vastly exceeding their traditional remit. These new powers do not fit well, however, with the independence of central banks, remote from the democratic control of government. Central Banking in Turbulent Times examines fundamental questions about the central banking system, asking whether the model of an independent central bank devoted to price stability is the final resting point of a complex development that started centuries ago. It dissects the hypothesis that the Great Recession has prompted a reassessment of that model; a renewed emphasis on financial stability has emerged, possibly vying for first rank in the hierarchy of objectives of central banks. This raises the risk of dilemmas, since the Great Recession brought into question implicit assumptions that the pursuit of price stability would also lead to financial stability. In addition, the border between monetary and fiscal policy was blurred both in the US and in Europe.
