Contexte : Compte tenu de la baisse du nombre de gynécologues et du suivi non optimal des femmes, les médecins généralistes jouent un rôle important en gynécologie de premier recours. Pourtant, l'examen peut être vécu comme inconfortable notamment par les hommes. Cette difficulté apparaît dès le début de la formation en gynécologie. Objectifs : Nous nous intéressons donc au ressenti des internes masculins lors de l'examen gynécologique puis à leurs attentes sur leur formation en gynécologie. Matériel et méthode : Étude qualitative inspirée de la théorisation ancrée à visée phénoménologique. L'analyse a porté sur dix entretiens semi-directifs auprès d'internes de Rennes ayant fait leur stage de gynécologie et leur SASPAS. Résultats : Le ressenti résulte de l'intrication de leurs représentations et des connaissances théoriques et pratiques mises en application lors de la relation particulière avec la patiente. L'encadrement pédagogique joue un rôle important. Le vécu durant leur formation conditionnerait leur intention de pratiquer la gynécologie plus tard. Ils attendent une formation professionnalisante, basée sur des objectifs d'acquisition de compétence : le savoir-faire et le savoir-être à l'aide d'un encadrement adapté à chaque niveau de maîtrise. Conclusion : Le vécu de l'examen gynécologique par les étudiants masculins dépend de plusieurs facteurs et pourrait conditionner leur pratique future. Une formation adaptée à la pratique de la gynécologie en médecine générale est donc nécessaire.

Introduction : Du fait de l'évolution de la démographie médicale, la pratique de la gynécologie tend à se développer en médecine générale. Elle requiert des praticiens formés et nécessite l'adaptation de la formation des internes de médecine générale. De plus, la profession de médecin généraliste se féminise. Dans ce contexte, quel est le devenir de la pratique gynécologique par les médecins généralistes masculins? Objectif : Evaluer le ressenti des internes masculins de médecine générale sur leurs formations en gynécologie durant l'internat, étudier les obstacles et proposer des améliorations. Matériel et méthode : Etude qualitative conduite par des entretiens semi-dirigés auprès d'internes masculins de la faculté de Grenoble ; avec une analyse thématique manuelle et une triangulation des données afin d'augmenter sa fiabilité. Résultats : La gynécologie fait partie des missions du médecin généraliste. Pour cela, les formations théorique et pratique des internes étaient primordiales. Cependant, la formation pratique paraissait hétérogène selon les terrains de stage. La séniorisation et le « debriefing » renforçaient la qualité de cette formation, tout comme la combinaison de terrains de stage hospitalier (accessibilité) et ambulatoire (influence du maître de stage). Les éléments limitant l'apprentissage de la discipline étaient le défaut d'accès aux consultations ainsi que l'exclusion des internes par les patientes. Conclusion : Cette étude montre un ressenti mitigé des internes masculins de médecine générale concernant leur apprentissage et leur pratique de la gynécologie durant l'internat. Des améliorations concernant l'encadrement, l'accessibilité aux consultations et la relation « interne-patiente » sont à développer.

L'accès à la consultation gynécologique est devenu un problème de santé publique en France. De nombreux médecins généralistes ne réalisent pas le suivi gynécologique de leurs patientes. Beaucoup de thèses abordent les difficultés rencontrées par les praticiens pour assurer les consultations gynécologiques en médecine de ville, notamment chez les médecins hommes. Ces derniers s'estimaient souvent sous-formés et moins consultés du fait de leur genre. Afin d'étayer cette hypothèse, nous avons réalisé une étude observationnelle par questionnaire auprès de 56 internes de médecine générale de la faculté de Caen. L'objectif était d'évaluer le ressenti des internes vis-à-vis de la formation reçue en gynécologie-obstétrique et d'évaluer s'il y avait des disparités d'accès à la formation en fonction du genre de l'interne. These guidelines have been developed to support countries to relieve pain in their paediatric populations. These guidelines address persisting pain in children caused by conditions such as cancer, HIV/AIDS, sickle-cell disease, burns, trauma, and phantom limb pain. Recommendations -- developed following a careful and transparent appraisal of available evidence -- are presented for the pharmacological treatment of mild, moderate and severe pain. The guidelines include chapters on the various systems used to classify pain and on the evaluation of pain, which reviews the available tools for routine pain measurement in children. Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.
