Antibiotic resistance is neither a surprising nor a new phenomenon. It is an increasingly worrisome situation, however, because resistance is growing and accelerating while the world's tools for combating it decrease in power and number. In addition, the cost of the problemâ€"especially of multidrug resistanceâ€"in terms of money, mortality, and disability are also rising. This book summarizes a workshop on antimicrobial resistance held by the Forum on Emerging Infections. The goal of the Forum on Emerging Infections is to provide an opportunity for representatives of academia, industry, government, and professional and interest groups to examine and discuss scientific and policy dilemmas of common interest that are specifically related to research on and the prevention, detection, and management of emerging infections. Organized as a topic-by-topic synthesis of presentations and exchanges during the workshop, the book highlights lessons learned, delineates a range of pivotal issues and the problems they raise, and proposes some simplified ideas about possible responses.

Antibiotics are drugs of natural or synthetic origin that have the capacity to kill or to inhibit the growth of micro-organisms. Antibiotics that are sufficiently non-toxic to the host are used as chemotherapeutic agents in the treatment of infectious diseases of humans, animals and plants. They have long been present in the environment and have played a crucial role in the battle between man and microbe. Many bacterial species multiply rapidly enough to double their numbers every 20-30 minutes, so their ability to adapt to changes in the environment and survive unfavourable conditions often results in the development of mutations that enable the species to survive changing external conditions. Another factor contributing to their adaptability is that individual cells do not rely on their own genetic resources. Many, if not all, have access to a large pool of itinerant genes that move from one bacteria cell to another and spread through bacterial populations through a variety of mobile genetic elements, of which plasmids and transposable elements are two examples. The capacity of bacteria to adapt to changes in their environment and thus survive is called resistance. Drug choices for the treatment of common infectious diseases are becoming increasingly limited and expensive and, in some cases, unavailable due to the emergence of drug resistance in bacteria and fungi - resistance that is threatening to reverse much medical progress of the past 50 years. Dissemination of resistant micro-organisms may occur in both hospitals and communities. It is recognized that a major route of transmission of resistant microorganisms from animals to humans is through the food chain. In aquaculture, antibiotics have been used mainly for therapeutic purposes and as prophylactic agents. The contribution to antimicrobial resistance of antibiotics used in aquaculture is reviewed here, using a risk analysis framework. Some recommendations on responsible conduct in this context are proposed, aimed at diminishing the threat of build up of antimicrobial resistance.

Antimicrobial resistance is a global and increasing threat. Stewardship campaigns have been established, and policies implemented, to safeguard the appropriate use of antimicrobials in humans, animals, and plants. Restrictions on their use in animal production are on the agenda worldwide. Producers are investing in measures, involving biosecurity, genetics, health care, farm management, animal welfare, and nutrition, to prevent diseases and minimize the use of antimicrobials. Functional animal nutrition to promote animal health is one of the tools available to decrease the need for antimicrobials in animal production. Nutrition affects the critical functions required for host defence and disease resistance. Animal nutrition strategies should therefore aim to support these host defence systems and reduce the risk of the presence in feed and water of potentially harmful substances, such as mycotoxins, anti-nutritional factors and pathogenic bacteria and other microbes. General dietary measures to promote gastrointestinal tract health include the selective use of a combination of feed additives and feed ingredients to stabilize the intestinal microbiota and support mucosal barrier function. This knowledge, used to establish best practices in animal nutrition, could allow the adoption of strategies to reduce the need for antimicrobials and contain antimicrobial resistance.
