Contexte - Le toucher est une forme de communication importante pour les professionnels de santé. Les patients racontent leurs expériences de toucher lors des soins et les médias les relaient. Le toucher est vécu différemment selon les acteurs concernés. Il fait partie des outils de communication non verbale du médecin généraliste (MG), participe à la construction de la relation patient-MG et a une valence technique pour l'examen physique, élément central de la consultation. Objectif - L'objectif de cette étude était de décrire et analyser les représentations et le vécu des patients face au toucher pratiqué par leur MG. Méthode - Étude qualitative par entretiens individuels semi-directifs utilisant la méthode par théorisation ancrée. Échantillonnage initial de convenance puis théorique. Corpus de données constitué de 21 entretiens, menés entre juin 2018 et mai 2019. Recrutement des participants dans des cabinets médicaux de MG d'Île-de-France. Analyse par comparaison constante, triangulée avec une chercheuse thésarde et une chercheuse senior. Résultats - Le toucher devait être certifié conforme aux attentes de chaque patient pour être accepté. Les patients interrogés identifiaient deux images en miroir du MG : l'une où il était maître de la situation de toucher et l'autre prestataire de services. Le MG était reconnu comme un proche par le patient, ce qui l'autorisait à le toucher. Notre étude avance que le MG sait trouver un juste milieu entre une attitude empathique dans l'usage du toucher et la prestation de services médicaux. Ces notions sont soumises à une certification de conformité aux attentes des patients et un besoin d'interagir entre êtres humains. Conclusion - Il semble que le toucher et son côté interhumain soulèvent un problème de société cristallisé autour de peurs. Le toucher du MG est un outil diagnostic. Sans examen physique, l'expertise médicale perd en efficience. Il serait intéressant de confronter les MG aux résultats de cette étude. Patients et médecins se rejoignent-ils sur le sens à donner au toucher ?
