Advances in epilepsy in recent decades have allowed for improved algorithms for diagnosis and a common understanding of terminology with the development of the International Classifications of Seizures and the Epilepsies. Nevertheless, no common system exists for the estimation of epilepsy severity or its impact on quality of life. Therefore, epileptologists lack the ability to make quantitative assessments of individual patients for comparison of care or for meta-analyses in clinical trials. This book on the Quantitative Assessment of Epilepsy Care approaches this omission by addressing the potential application of clinimetrics within the framework of epilepsy treatment. Clinimetrics is a fast growing discipline concerned with the quantification of clinical symptoms with respect to decision making relating to diagnosis, treatment, and prognosis. These methods allow for the development and validation of clinical scoring systems. For example, the Glasgow Coma Scale is widely used. As a chronic disorder, epilepsy would benefit from clinimetric methodology to create uniformity and to allow for comparisons among evaluations. In addition, epileptologists have not yet developed assessments of health related quality of life to define the overall condition of the chronic epilepsy patient and various therapeutic endpoints. While clini metric tools are essential for research, they will also be useful in clinical practice for the care of individual patients by documenting status and changes over time. This treatise will provide critical analyses of whether existing rating scales and techniques are valid to use, and which types of scales and techniques require further development.

Epilepsy, a chronic neurological disorder, affects over 50 million individuals worldwide. Thirty percent of these have seizures that are not amenable to medications, and of these, at least one-half may be candidates for surgical treatment. While epilepsy surgery maybe an option with high rates of seizure freedom, this does not necessarily guarantee a good psychosocial outcome. Despite well documented issues of post-operative adjustment difficulties, little information exists regarding post-surgical epilepsy patients and what they believe would have benefited them post-surgically, if they had received it pre-surgically. Therefore, the purpose of this study was to explore possible knowledge gaps around the surgical event. This qualitative needs assessment study involved interviewing post-surgical epilepsy patients. The results suggest pre-surgical information and support gaps exist and require change. Furthermore, in conjunction with our recently proposed predictive coding model of post-epilepsy surgery adaptation, our results provide the groundwork for developing a pre-operative intervention to address patient predictions and expectations.

In recent decades, neurologists have been directing more of their research efforts to exploring the sources of health disparities in medical and surgical treatment of epilepsy. Many studies reveal that racial and ethnic minority patients continue to receive suboptimal care, which has some dire repercussions on their physical and mental health, as well as their social well-being because epilepsy is a chronic disease that tends to affect multiple aspects of the patient's life. Although the earliest landmark studies emphasized the importance of mixed methodology research, the studies that followed tended to rely heavily on quantitative methods to unravel patterns of disparities with sparse use of qualitative methods to give voice to the patients concerned. In this work, I present a mixed methodology framework that is particularly suitable to investigating health disparities in epilepsy care, which affirms the complementary nature of quantitative and qualitative methods. I explore some of the challenges that clinicians face to utilizing qualitative methods, and introduce some of the validity criteria and techniques of qualitative research that make it a valuable methodology to understand disparities. I highlight some of the ethical concerns with recent studies in health disparities in epilepsy care which adopt only quantitative or qualitative methodology, and contribute very little to eliminating disparities compared to the potential contribution of mixed methodology research. This will be supported by various examples from research led by clinicians, public health professionals, and social scientists.
