L’imagerie de perfusion a démontré son intérêt pour le diagnostic et le suivi thérapeutique des tumeurs cérébrales. Les méthodes d’IRM permettant de mesurer la perfusion peuvent être divisées en deux groupes : les méthodes utilisant des agents de contraste intraveineux et celles qui n’en utilisent pas. Les applications cliniques concernant cette modalité d’imagerie sont encore en évaluation. Après des rappels sur les tumeurs cérébrales, sur l’angiogenèse et les propriétés des produits de contraste, il nous est apparu intéressant de faire le point sur les principales techniques d’imagerie par résonance magnétique de perfusion (IRM), de décrire les différentes séquences d’acquisition et les méthodes d’analyse des images de perfusion. Dans une seconde partie trois études cliniques réalisées en imagerie de perfusion de premier passage, en perméabilité et en perfusion sans injection de produit de contraste sont détaillées, et permettent de caractériser différents types de tumeurs cérébrales (e.g., méningiomes, glioblastomes). Ces études démontrent bien l’intérêt clinique d’utiliser l’IRM de perfusion pour un meilleur diagnostic des lésions encéphaliques.
