Les diarrhées aiguës (DA) fréquentes, coûteuses et encore graves représentent un véritable enjeu de santé publique. C'est en effet la première cause de décès évitables chez les enfants de moins de 5 ans qui sont liés à la déshydratation par non utilisation des solutions de réhydratation orale (SRO) qui malgré leur efficacité, leur simplicité d'utilisation et leur faible coût sont insuffisamment utilisées. Le Conseil Supérieur d’Hygiène publique de France constate dans son avis relatif à la vaccination antirotavirus chez les nourrissons de moins de 6 mois (séance du 22/09/2006), « qu’au regard de l’épidémiologie, les modes de prise en charge en France des diarrhées de l’enfant doivent encore être améliorés » et « recommande dans un premier temps, de mettre en œuvre un programme de suivi de ces actions et une étude prospective du suivi d’évolution des pratiques concernant la prise en charge des GEA et la réhydratation orale. ». Objectifs de l’étude : évaluer la prise en charge pré-hospitalière des DA chez les nourrissons admis aux urgences pédiatriques (UP) de Nantes en Loire-Atlantique entre janvier et avril 2008 selon 2 axes : la prise en charge thérapeutique et en particulier la prescription de SRO par les médecins libéraux ; et les motifs de venue aux UP pour une DA. Méthodes : étude prospective aux UP de Nantes pendant 4 mois (de janvier à avril 2008) à partir d’un questionnaire distribué aux examinateurs médicaux par les infirmières lors d’un cas de DA. Le nombre de cas inclus a permis d’appliquer la loi des grands nombres et les théorèmes de statistique inférentielle à partir desquelles les intervalles de confiance à 95% ont été déterminés. Chaque cas clinique peut être considéré comme un tirage aléatoire dans la population de référence. Résultats : 105 nourrissons ont été inclus. 77% des médecins libéraux (63% de généralistes et 10% de pédiatres) prescrivent des SRO lors d’une DA du nourrisson mais 76% prescrivent encore d’autres médicaments. Les médicaments les plus prescrits sont les anti-émétiques dans 48% des cas et les antisécrétoires intestinaux dans 37% des cas. 64% des parents rencontrent des difficultés lors de l’utilisation des SRO essentiellement liées aux vomissements et au refus des SRO. Ces difficultés augmentent avec l’âge de l’enfant et leur présence double le risque d’hospitalisation par rapport à l’absence de difficultés. Les conseils doivent obligatoirement accompagner la prescription puisqu’ils permettent de réduire les difficultés et le taux d’hospitalisation qui est de 24% lorsque les conseils ont été complets à 80% lorsque les SRO ont été prescrits sans conseils. Enfin seuls 50% des nourrissons sont adressés par un médecin même si 81% ont déjà vu un médecin avant leur admission. Or 25% des nourrissons admis sans avis médical seront hospitalisés alors que 50% des nourrissons admis sur avis médical le seront. Ces données encouragent à respecter la filière de soins où le médecin généraliste libéral est le médecin de premier recours. Enfin, il faut noter la discordance entre l’urgence ressentie par les parents et les critères de gravité médicaux qui contribue à augmenter le nombre de consultation aux UP. Cette étude n’a malheureusement pas permis d’explorer la prise en charge nutritionnelle des nourrissons lors des DA faute de réponses suffisantes. Conclusion : Ces données sont positives et montrent un bon taux de prescription de SRO malgré la prescription persistante d’autres médicaments. L’éducation thérapeutique des parents est primordiale ainsi que le respect de la filière de soins pour diminuer l’impact des DA sur la santé publique. Il faut aussi insister au niveau des professionnels de santé sur l’importance qu'ont les conseils accompagnant la prescription de SRO sur la réussite du traitement. Après toutes ces mesures, la place reste pour innover sur la formule du SRO et définir une population cible pour la vaccination contre le rotavirus.

La gastroentérite aiguë est une pathologie fréquente chez le nourrisson, responsable d'une morbidité et d'un coût important en France. Son traitement est actuellement bien codifié. L'objectif est de réévaluer les pratiques médicales en 2008, en analysant la prise en charge pré hospitalière de nourrissons consultant aux urgences pour diarrhées. La méthode utilisée est l'étude d'observation transversale réalisée à l'hôpital Ambroise Paré (Boulogne) du 1er novembre 2007 au 1er mars 2008, qui a inclus les nourrissons âgés de 6 à 24 mois consultant aux urgences pour diarrhées. Les résultats sont les suivants : parmi les 212 nourrissons, 34% avaient consulté un médecin au préalable. Les médecins ont prescrit du SRO dans 71% des cas : 64% pour les médecins généralistes et 91% pour les pédiatres [p=0.0026]. Les médicaments adjuvants de la diarrhée les plus prescrits étaient le racécadrotril (64%) et le diosmectite (19%). Les pédiatres prescrivaient du racécadotril dans 84% des cas conte 45% pour les médecins généralistes [p=0.0002]. Aucun traitement antibiotique à visée antidiarrhétique n'a été prescrit. Le changement de lait était préconisé par 15% des médecins généralistes et 34% des pédiatres (NS). Notre étude montre dons une amélioration des pratiques par rapport aux études précédentes aussi bien en terme d'accroissement de prescription de SRO et de racécadotril qu'en terme de diminution des médicaments adjuvants de la diarrhée ne rentrant pas dans le cadre des recommandations. Néanmoins, les diarrhées sont encore responsables d'une morbidité et d'un coût trop important en France. Une amélioration dans la diffusion des recommandations aux médecins et dans l'éducation des familles est souhaitable.

It highlights continued progress made towards meeting international targets for malaria control to be achieved by 2010 and by 2015. International funds disbursed for malaria control increased from $200 million in 2004 to $1.5 billion in 2009. Since 2008, more than 289 million insecticide-treated mosquito nets have been delivered to sub-Saharan Africa, enough to protect three quarters of the 765 million persons at risk of the disease. Over the last decade, 11 countries in the Africa Region and 32 countries in other regions showed reductions of 50% or greater in either confirmed malaria cases or malaria admissions and deaths. Malaria control is making a major contribution to reducing mortality in children less than 5 years old and to attaining the health-related Millennium Development Goals.

Not everyone is a friend of the manifold abbreviations that have by now beCome a part of the scientific language of medicine. In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.

Having safe drinking water is important to all Americans. The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.

In addition, because family planning programmes often significantly affect population growth, the study examines the impacts of family planning on fertility and health, and the human rights implications of family planning programmes. In considering the book's conclusions about the impact of population growth on development, four caveats should be noted. First, the effects of population growth vary from place to place and over time. Thus, blanket statements about overall effects often cannot be made. Where possible, the authors note the contexts in which population effects are strongest and weakest. Second, all of the outcomes examined in this book are influenced by factors other than population growth. Moreover, the impact of population growth may itself vary according to the presence or absence of other factors. This again makes bl anket statements about the effects of population growth difficult. Throughout the chapters, the authors try to identify other relevant factors that influence the outcomes we discuss or that influence the impact of population growth on those outcomes.
