La gestion quotidienne de la maladie et la prévention des complications sont notamment médiées par l' « adhésion » de la personne à sa prise en charge. Cette dernière peut être modulée par différents facteurs inhérents à la personne, dont la présence de troubles psychiatriques ou psychologiques. L'objectif de ce travail était de déterminer si des facteurs psychiatriques et psychologiques étaient associés à la présence de plaie ou de récidive de plaie chez les personnes diabétiques, et de comparer la fréquence de ces troubles entre les personnes diabétiques hospitalisées et celles en population générale. Méthodes : L'étude DIAPEPSY est une étude dicentrique menée aux CHU de Montpellier et du Grau du Roi. Les personnes diabétiques de plus de 45 ans avec ou sans plaie du pied ont été recrutéesentre décembre 2016 et juillet 2019. Une évaluation de l'état psychiatrique et psychologique a été réalisée par auto-questionnaires, complétée par un questionnaire de données médicales. Les données des personnes de plus de 65 ans ont ensuite été comparées à celles de l'étude ESPRIT, menée en population générale. Résultats : 151 personnes sans plaie du pied et 93 personnes avec ont été inclues. On ne retrouvait pas de différence significative entre les 2 groupes concernant les principaux troubles psychiatriques et psychologiques, hormis un nombre de tentative de suicide plus élevé chez les témoins (15,3% vs 5,5% , p=0,021). Chez les personnes diabétiques de plus de 65 ans, la prévalence de la dépression, de l'anxiété, et des troubles bipolaires était significativement plus importante dans le groupe DIAPEPSY qu'en population générale (CES-D à 16,16 vs 12,92 p=0,001, Spielberger à 34,72 vs 29,94, p

In this book, Lawrence J. Brown offers a contemporary perspective on how the mind transforms, and gives meaning to, emotional experience that arises unconsciously in the here-and-now of the clinical hour. Brown surveys the developments in theory and practice that follow from Freud’s original observations and traces this evolution from its conception to contemporary analytic field theory. Brown emphasizes that these unconscious transformational processes occur spontaneously, in the blink of an eye, through the "unconscious work" in which the analyst and patient are engaged.
