A little bit more than two years after the first edition, we are pleased to publish the second edition of this book. More than half of the chapters have been remodeled and completed as the result of technical improvements and recently acquired clinical data. During these two years, the number of coronary pressure measurements performed worldwide both during diagnostic and interventional procedures has increased almost exponentially. Most of the initial problems associated with this new approach have been overcome. Many colleagues have explored new research avenues and in many catheterization laboratories the method has matured from a research toy to a clinical tool. Classical indications such as the intermediate stenosis and guidance of PTCA or stent implantation, have been largely extended and coronary pressure measurement has proven to be useful in multi vessel disease, diffuse disease, long and serial stenosis, after myocardial infarction, and in many other diagnostic and interventional situations encountered in the catheterization laboratory. Quite unexpectedly, this approach has also enforced the ties with our surgical colleagues in the selection of patients suitable for minimal invasive surgery or hybride revascularization. Also in mild and intermediate left main disease, there is a role for coronary pressure measurement in the process of decision making. High pressure has become a basic variable in many areas of science and engineering. It extends from disciplines of geophysics and astrophysics through chemistry and physics to those of modern biology, electrical and chemical engineering. This breadth has been recognized for some time, but it was not until the early 1960's that an international group of scientists and engineers established the Association Internationale for Research and Advancement of High Pressure Science and Technology (AIRAPT) for bringing these various aspects of high pressure together at an international conference. The First AIRAPT International High Pressure Conference was held in 1965 in France and has been convened at approximately two to three year intervals since that time. The past four AIRAPT International High Pressure Conferences have been held in Germany, Scotland, Japan and the U.S.S.R. In the past decades, lightweight building construction methods and the use of manmade materials in construction and furnishings have become more and more common. The time until structural failure can be expected in a fire has been reduced, and firefighters have seen hotter fires that generate high levels of deadly gasses. But the ventilation methods used by modern firefighters have not kept pace. Positive pressure was first used in the fire service to ventilate a structure after the fire was knocked down. Authors Kriss Garcia and Reinhard Kauffmann have taken positive pressure a step further to achieve effective ventilation in coordination with aggressive fire attack, called positive pressure attack (PPA). Properly used PPA allows firefighters great control over the interior environment of a fire building, and starts at the earliest stages of the operation when ventilation can provide the greatest benefit for victims, firefighters, and the structure.
