This review of China's health sector begins by noting China's achievements in population control, health status, and nutrition. The policies, sources of financing, and resource use that have contributed to China's successes are cited, and the evolution of several influences on health from outside the sector are discussed, including nutritional improvements, greater access to clean water and sanitary waste disposal, and fertility reduction. Two major challenges now face China's health sector: (i) extending the methods that have been successful to areas where mortality rates and deaths due to infectious diseases remain high; and (ii) developing and implementing approaches to management of chronic disease that combine prevention, low-cost treatment, rehabilitation, and humane care. Major innovation will be essential because resources are limited and plans for meeting the health needs of the population are ambitious. Diverse driving forces, processes and actors are responsible for different trends in the development of megacities and large urban areas. Under the dynamics of global change, megacities are themselves changing: On the one hand they are prone to increasing socio-economic vulnerability due to pronounced poverty, socio-spatial and political fragmentation, sometimes with extreme forms of segregation, disparities and conflicts. On the other hand megacities offer positive potential for global transformation, e.g. minimisation of space consumption, highly effective use of resources, efficient disaster prevention and health care options – if good strategies were developed. At present in many megacities and urban areas of the developing world and the emerging economies the quality of life is eroding. Most of the megacities have grown to unprecedented size, and the pace of urbanisation has far exceeded the growth of the necessary infrastructure and services. As a result, an increasing number of urban dwellers are left without access to basic amenities like clean drinking water, fresh air and safe food. Additionally, social inequalities lead to subsequent and significant intra-urban health inequalities and unbalanced disease burdens that can trigger conflict and violence between subpopulations. The guiding idea of our book lies in a multi- and interdisciplinary approach to the complex topic of megacities and urban health that can only be adequately understood when different disciplines share their knowledge and methodological tools to work together. We hope that the book will allow readers to deepen their understanding of the complex dynamics of urban and megacity populations through the lens of public health, geographical and other research perspectives.

Why another book about vaccines? There are already a few extremely well-written medical textbooks that provide comprehensive, state-of-the-art technical reviews regarding vaccine science. Additionally, in the past decade alone, a number of engrossing, provocative books have been published on various related issues ra- ing from vaccines against specific diseases to vaccine safety and policy. Yet there remains a significant gap in the literature – the history of vaccines. Vaccines: A Biography seeks to fill a void in the extant literature by focusing on the history of vaccines and in so doing, recounts the social, cultural, and scientific history of vaccines; it places them within their natural, historical context. The book traces the lineage – the “biography” – of individual vaccines, originating with deeply rooted medical problems and evolving to an eventual conclusion. Nonetheless, these are not “biographies” in the traditional sense; they do not trace an individual’s growth and development. Historically, governments have played the predominant role in owning and operating infrastructure facilities such as schools, hospitals, roads, bridges, railways, ports, telecommunications networks, and water and electricity supply facilities. However, fiscal policy constraints, growing acceptance of the user-pays principle, and a recognition that there are generally greater incentives for efficiency in the private sector, have driven increased private involvement in the provision of both economic and social infrastructure. A new Commission Staff Working Paper reports on the experiences of a number of countries using different approaches to funding public infrastructure projects. The countries covered in the study are Australia, Canada, France, Germany, New Zealand, Sweden, the United Kingdom and the United States. In most countries, general government investment in infrastructure has declined in recent years. Nevertheless, overall investment in infrastructure has remained fairly steady, although volatile in some countries. Total Australian investment in infrastructure was just below 6 per cent of GDP in 2006-07. Sub-national governments undertook 76 per cent of public infrastructure investment, with government trading enterprises (GTEs) accounting for around half of this.
