Introduction : le diagnostic de maladie d'Alzheimer (MA) n'est actuellement posé que tard, au stade démentiel. Un premier pas vers un diagnostic précoce se base sur la classe diagnostique Mild Cognitive Impairment (MCI), regroupant les sujets atteints de troubles cognitifs évocateurs de MA mais encore trop légers pour affirmer une MA. L'objectif de cette étude est d'évaluer la capacité des biomarqueurs (Imagerie par Résonance Magnétique ou IRM, Tomographie par Emission de Positons ou TEP, analyse du Liquide CéphaloRachidien ou LCR) à prédire au niveau individuel l'évolution clinique des sujets MCI. Matériel et méthode : l'étude, basée sur une cohorte multicentrique, incluait les sujets MCI âgés de 60 à 85 ans dont le suivi était supérieur à 18 mois. Cette population fut scindée en 2 sous-classes en fonction du diagnostic final : stable MCI (sMCI) pour les sujets qui étaient restés MCI, et progressive MCI (pMCI) pour ceux qui avaient évolué vers une MA. Nous avons évalué la capacité des scores neuropsychologiques (MMSE, ADAS-COG, AVLT) et des biomarqueurs, mesurés à l'inclusion, à prédire le statut final (sMCI ou pMCI). L'évaluation a été menée au niveau populationnel (Hazard Ratio ou HR) et au niveau individuel (précision de prédiction). Résultats : 979 sujets MCI ont été inclus, leur suivi moyen était de 35 mois. En fin d'étude, 321 avaient développé une MA, 658 étaient restés stables. Les HR des biomarqueurs (4,45 pour le PET FDG, 4,61 pour l'IRM, 5,0 pour le LCR) étaient tous supérieurs à ceux des scores neuropsychologiques. Au niveau individuel, l'IRM et le TEP FDG montraient la plus haute précision (73%). La meilleure précision était atteinte par combinaison de l'ensemble des biomarqueurs et scores neuropsychologiques (80%). Conclusion : les biomarqueurs, en conjonction avec les scores neuropsychologiques, prédisent à 80% l'évolution clinique future vers une MA dans les 35 mois chez les sujets MCI.
