Introduction : la santé physique et la surmortalité des personnes vivant avec des maladies psychiques ont longtemps été ignorées, du fait de préjugés, de méconnaissances ou de difficultés de dépistage. Pourtant, certains médicaments psychotropes augmentent les risques de maladies cardio-vasculaires, de diabète et d'obésité, et de nombreuses études cliniques et épidémiologiques ont porté sur l'association entre maladies somatiques et psychiques. Tous ces facteurs retardent ou limitent l'accès à des soins adaptés, diminuent de manière importante l'espérance de vie des patients et compliquent la prise en charge de leur santé mentale et somatique. Afin d'optimiser la prise en charge globale, une meilleure coordination entre psychiatres et somaticiens, psychiatrie et médecine de ville, est donc indispensable. Cela doit notamment passer par la formation initiale. L'objectif de cette thèse est d'étudier la pertinence du stage de soins somatiques en établissement psychiatrique pour les internes en médecine générale. Méthodes : une étude qualitative par théorisation ancrée, par entretiens semi-structurés, individuels, a été réalisée a à partir d'un guide d'entretien auprès d'internes passés en stage de soins somatiques sur le GHU Paris psychiatrie et neurosciences. L'enregistrement a été audio avec transcription verbatim des données. Analyse et résultats : recueil de 13 entretiens. Il en ressort un intérêt évident des internes de médecine générale pour la psychiatrie, une méconnaissance de la pratique des soins somatiques, un certain regard posé sur le patient suivi en santé mentale, des difficultés face à la prise en charge des situations complexes rencontrées en psychiatrie, un stage encadré avec une autonomie rapide de l'interne, une réflexion éthique sur une population vulnérable, l'importance d'une formation universitaire adaptée. Discussion : les médecins généralistes bénéficient au cours de leur parcours universitaire d'une formation à la psychiatrie qui apparaît insuffisante : intérêt d'ouvrir au niveau national des stages de soins somatiques en établissement psychiatrique pour mieux sensibiliser les étudiants, mais aussi de mettre en place des formations de Développement Professionnel Continu (DPC) sur le suivi somatique en santé mentale pour les médecins généralistes. La prise en charge somatique ambulatoire des patients suivis en santé mentale doit être améliorée, et il persiste un problème crucial de communication entre soignants.

Introduction : 25% de la population mondiale est concernée à un moment de sa vie par un trouble mental. Le Médecin Généraliste est le médecin de premier recours dans la population générale et sa formation en psychiatrie parait insuffisante. Lors du Diplôme d'Etude Spécialisé de médecine générale, les stages obligatoires bénéficient d'une liste d'objectifs régie par le département de médecine générale (DMG). Afin de promouvoir la formation de l'Interne de Médecine Générale (IMG) en psychiatrie et d'améliorer sa pratique future, ce travail avait pour objet d'établir une liste de compétences à acquérir lors de la réalisation du stage en psychiatrie ; stage libre, à Lille. Méthode : Qualitative de consensus, par groupe nominal, réalisé à Lille, et regroupant des experts en psychiatrie. Les chefs de service de psychiatrie recevant des IMG ainsi que les psychiatres participant aux réunions de la Commission Médicale d'Etablissement (CME) ont été invités. Résultats : 39 chefs de service de psychiatrie et 30 psychiatres présents à la CME de psychiatrie ont été invités par mail. 12 réponses sont parvenues favorables. Le groupe nominal a finalement regroupé 10 experts en psychiatrie. Un travail initial de réflexion individuel a permis de formuler 92 items. Une harmonisation encadrée par un modérateur a abouti à une liste de 45 items. La hiérarchisation a été organisée par vote des experts attribuant une note de 1 à 10 par ordre de priorité aux 10 items leurs semblant les plus importants. Cette étape s'est déroulée par échange de mails aboutissant à une liste de 12 objectifs notés de 72 à 17. Conclusion : Cette étude conclut à une liste de 12 compétences que l'IMG doit acquérir lors de la réalisation de son stage en psychiatrie. A charge du DMG et des enseignants d'en définir la place au sein de la formation.
