Dans le bilan d'extension des cancers de la cavité buccale, parmi les différentes modalités d'imagerie mandibulaire actuellement utilisées en pratique clinique courante, aucune n'apporte la confirmation de l'atteinte osseuse mandibulaire, et, dans un tel cas, les limites précises de cette atteinte. L'objectif de ce travail de thèse est d'étudier cette problématique, et de proposer au clinicien une méthode de visualisation et de délimitation de ces atteintes osseuses mandibulaires. Ce travail repose sur l'utilisation des images fonctionnelles de tomographie par émission de positons (TEP) couplées aux images anatomiques scanner X (images hybrides TEP/CT). Alors que le radiopharmaceutique à ce jour le plus utilisé dans un bilan d'extension d'un cancer est le 18F-FDG, nous avons proposé dans le cadre de cette thèse d'évaluer un marqueur osseux plus spécifique, le 18F-NaF. Pour l'analyse quantitative des images un logiciel spécifique de visualisation et de traitement des images mandibulaires de TEP/CT au 18F-FDG et au 18F-NaF a été développé puis évalué dans le cadre de cette thèse. Ce logiciel comporte plusieurs paramètres de réglage. L'étude de ces paramètres a nécessité un travail à la fois sur données physiques et sur des données cliniques de patients. La première étape dans l'évaluation du logiciel fut sur pièce osseuse mandibulaire, excluant tout parasitage par une activité radioactive osseuse d'origine non mandibulaire. Nous avons montré que le logiciel est capable de fournir des mesures précises, non déformées, sur une image linéaire de la mandibule. Il a également permis de confirmer que le référentiel spatial utilisé est suffisamment fiable pour pouvoir l'utiliser lors d'une étude clinique. L'évaluation clinique repose sur 15 patients ayant subi une mandibulectomie. La délimitation par le logiciel de l'extension tumorale osseuse, mesurée à partir des images TEP/CT au 18F-FDG et au 18F-NaF de ces patients avant exérèse, a été comparée aux données issues des analyses histologiques des pièces de mandibulectomie. Cette étude a permis, d'une part de comparer les deux radiopharmaceutiques, et d'autre part de proposer les valeurs de réglages du logiciel permettant d'optimiser son fonctionnement en vue d'une utilisation en pratique clinique quotidienne. En conclusion, cette thèse a permis de valider une nouvelle approche en imagerie mandibulaire des cancers de la cavité buccale, en montrant d'une part que le radiopharmaceutique de choix dans la quantification de l'atteinte osseuse mandibulaire d'un cancer de la cavité buccale est le 18F-NaF, et en montrant d'autre part qu'un logiciel bien paramétré est capable de délimiter avec suffisamment de précision les limites de l'exérèse. Enfin, plusieurs perspectives de recherches sont envisagées en conclusion de ce travail.
