Excerpt from The New York Medical Journal, Vol. 40: A Weekly Review of Medicine; July to December, 1884, Inclusive I recently saw a case which presented the features of a hybrid general paresis with bulbar, and finally very marked and persistent renal symptoms, the amount of urine voided daily exceeding twenty ounces at times, while at others it was scanty, but contained albumin or sugar at all times. During the past six weeks the patient's legs and feet have been the seat of a puffy swelling, the skin being red and glossy; and there was very slight pitting on pressure, which disappeared in the course of a few days. The patient's dis ease began with embarrassment in articulation, pupillary alteration, tremor of the tongue, and finally of the whole body, and then came the delusions of grandeur, which were of the most extravagant nature, and he became salacious in the extreme. Nearly all of the cranial nerves were affected. There were facial anaesthesia, impairment of smell and taste, and very curious vaso-motor changes, which consisted mainly in a discoloration of the skin of the left arm and hand. From time to time I found sugar in his urine, and this seemed to be the case after an attack of excitement. With the kind assistance of Dr. Landon, of the Hudson River State Hospital for the Insane, I am enabled to present the records of urinary analyses in twelve cases of general paresis of the insane, which were made every day for fifteen days. In these patients the bulbar symptoms were marked and the urine was increased in quantity. In all there was a varying amount of sugar. It may be stated that these patients were upon the same diet and treated precisely alike, so the results have more value than if the cases were selected at random. Of these twelve cases, it will be seen that in two there was considerable sugar, and in one case there had been some months previously a large quantity found. In Case 6 both albumin and sugar were found. On ten occasions the former was present, and on five absent. It is to be noted that, as a rule, when albumin was present the sugar was not to be found, and nice versa. The quantity of sugar seemed to bear no important relation to the quantity of urine, and on days when no sugar was found the patient often passed from 50 to 60 oz.
