Les modèles déformables tels que les contours actifs sont des outils généraux et puissants pour la segmentation, en permettant notamment l'adjonction de contraintes et de connaissances a priori sur les objets à segmenter. Un modèle déformable est une structure géométrique déformée à l'aide d'une méthode d'évolution afin de s'ajuster aux frontières des objets recherchés. La segmentation est formulée comme un problèpmme d'optimisation, le but étant de déterminer la courbe ou la surface minimisant une fonction objectif (une énergie) composée de termes internes relatifs à la régularité géométrique du modèle et de termes externes qui mettent en relation le modèlle et l'image. Dans cette thèse, nous développons un modèle de contour actif pour la segmentation 2D et un modèle de surface active pour la segmentation 3D, ces deux modèles étant basés sur un formalisme unifié. Nous généralisons également le modèle de surface au problème de segmentation et suivi 3D+T. Nous proposons un ensemble d'amélioration de l'algorithme glouton, une méthode de minimisation numérique de la fonctionnelle d'énergie. Nous développons un nouveau terme externe basé région, que nous appelons énergie de région en bande étroite. Ce terme combine des caractéristiques locales et globales des structures d'intérêts et présente des avantages tant au niveau calculatoire qu'au niveau de la cohérence par rapport aux données.
