This paper critically reviews the methods for measuring the impact of family planning programs on fertility used primarily in the last decade, based on the literature as well as site visits and interviews. The strengths and weakness of several acceptor-based and population-based methods are evaluated for use in particular settings. The acceptor- based methods include: reproductive process analysis, standard couple- years of protection (SCYP), CYP, individual matching and component projection (CONVERSE). The population-based methods are: decomposition (standardization), trend analysis, prevalence, areal regression, multilevel regression and experimental design. Current evaluations are focusing on identification of the aspects of programs that have an impact, their interaction, and their relative importance in different settings. Except for the use of CYP in several Asian programs to monitor performance of the program, population-based methods are in vogue recently. Reasons for their popularity are the availability of national survey data, microcomputers and software such as TARGET, and the need for target setting in country policy. Areal and multilevel regression and experimental design are the methods receiving the most attention. The success of the Matlab project has stimulated interest in experimental design.

This paper addresses the key elements and changes in population policies and programs, including the strength of program effort, from 1972 to 1982. Using a new scale with thirty items to measure family planning program effort, the presentation is based on an analysis of over 300 questionnaires received from ninety-three countries. Attention is given to measurement problems and change in program effort 1972-82 (including findings by country and regional differences). This study shows that a great deal of family planning program effort exists in a small number of countries; moderate effort occurs in a larger number of countries; and weak or very little effort is found in an even greater number of countries, including many in the Middle East, North Africa and sub-Saharan Africa. Concerning change over time, between 1972 and 1982, there was a modest increase in program effort in more than half of the nearly 100 countries studied and a substantial increase in program effort in more than a third.

This volume is part of an effort to review what is known about the determinants of fertility transition in developing countries and to identify lessons that might lead to policies aimed at lowering fertility. It addresses the roles of diffusion processes, ideational change, social networks, and mass communications in changing behavior and values, especially as related to childbearing. A new body of empirical research is currently emerging from studies of social networks in Asia (Thailand, Taiwan, Korea), Latin America (Costa Rica), and Sub-Saharan Africa (Kenya, Malawi, Ghana). Given the potential significance of social interactions to the design of effective family planning programs in high-fertility settings, efforts to synthesize this emerging body of literature are clearly important.

A Review of the HHS Family Planning Program provides a broad evaluation of the Title X family planning program since its establishment in 1970. The program successfully provides family planning services to its target audience of low-income individuals, but there is room for improvement. While the program's core goals are apparent, a secondary set of changing priorities has emerged without a clear, evidence-based strategic process. Also, funding for the program has increased in actual dollars, but has not kept pace with inflation or increased costs. Several aspects of the program's structure could be improved to increase the ability of Title X to meet the needs of its target population. At the same time, the extent to which the program meets those needs cannot be assessed without a greater capacity for long-term data collection. A Review of the HHS Family Planning Program recommends several specific steps to enhance the management and improve the quality of the program, as well as to demonstrate its direct contribution to important end results, such as reducing rates of unintended pregnancy, cervical cancer, and infertility. The book will guide the Office of Family Planning toward improving the effectiveness of the program. Other parties who will find the research and recommendations valuable include programs receiving Title X funding from the Office of Family Planning, policy makers, researchers, and professional organizations.
