Contexte : La prescription des opioïdes forts dans le traitement des douleurs chroniques non cancéreuses est encadrée depuis peu, notamment par une mise au point de l'AFSSAPS de 2004, mais cette indication fait toujours débat. Objectifs : Mon étude a tenté de décrire les connaissances théoriques des médecins généralistes et leurs pratiques de prescription des opioïdes forts dans ce contexte, en les confrontant aux recommandations. Méthode : Enquête par questionnaire réalisée auprès de 79 praticiens au cours de quatre sessions de formation professionnelle. Le questionnaire comportait 9 vignettes et 31 questions sur la prescription des opioïdes forts. Résultats : La participation a été de 55,7%. 57,5% des praticiens jugeaient leur formation initiale au maniement des opioïdes forts inadaptée. La notion d'un seuil amenant à reconsidérer la prescription était peu connue, les recommandations sur les modalités de sevrage semblaient relativement ignorées. Cependant, les médecins affirmaient prescrire assez facilement des opioïdes forts : 92,7% pourraient les prescrire au long cours. À noter que le suivi de toxicomanes aux opiacés, une meilleure accessibilité aux structures « douleur » et une formation spécifique au maniement des opioïdes forts semblaient associés à une prescription des opioïdes forts plus aisée et / ou plus conforme aux recommandations. Conclusion : Bien que d'indication récente, le recours aux opiacés forts dans les douleurs chroniques non cancéreuses suscite peu de réticences pour les médecins généralistes. Cette prescription, parfois peu encadrée, pourrait poser problème lorsque le risque iatrogène appelle à la prudence, notamment chez le sujet âgé ou en cas d'antécédents psychiatriques ou d'éthylisme.
