Introduction : La grossesse a été identifiée comme un facteur majeur de prise de poids à long terme pour les femmes : Est-il encore pertinent de déconseiller les régimes amaigrissants aux femmes allaitantes ? Méthode : Il s'agit d'une revue de la littérature concernant les régimes amaigrissants pendant l'allaitement maternel, chez les femmes d'Europe, pourtour méditerranéen, Amérique du Nord et du Sud, Asie et Océanie, à partir des thèses du SUDOC, des revues de littérature de la Cochrane Library, de la littérature grise, des recommandations françaises et anglosaxonnes, et des articles médicaux issus des revues médicales ou du site Pubmed. Résultats : 61 documents ont été inclus, dont 24 articles scientifiques et 11 recommandations de bonne pratique. En cas de régime hypocalorique, la production de lait semble dépendre des réserves adipeuses de la mère. Les régimes restrictifs ont été peu étudiés mais il semble que les taux de protéines et de lactose dans le lait soient conservés. Les concentrations en micronutriments impliqués dans la croissance du nourrisson (vitamines A, B6, B12 et E, iode, sélénium et les acides gras polyinsaturés à chaines légères) dans le lait sont rapidement affectés par l'alimentation maternelle. Conclusion : Les données scientifiques rassemblées ne parviennent pas à nous rassurer quant à l'innocuité d'un régime amaigrissant sur la production ni sur la qualité du lait.
