Le Lean health care est un courant managérial, introduit dans le réseau de la santé et des services sociaux, qui vise l'optimisation des soins et la réduction du cout des services. Dans le contexte de lutte aux déficits publics qui perdurent depuis la fin des années 1980, le modèle Lean apparait comme une solution intéressante à la réduction des couts d'exploitation du réseau. Ce mémoire de maitrise cherche à analyser l'état actuel du climat et de la qualité du travail dans les CSSS, en regard du contexte d'implantation du modèle Lean. La collecte de données résulte de la distribution d'un questionnaire aux membres de l'Alliance du personnel professionnel et technique de la santé et des services sociaux (APTS) travaillant dans deux (2) différents Centres de santé et des services sociaux des régions de Montréal et de Gatineau. Il apparait que l'introduction d'une telle logique d'optimisation engendre des modifications dans la gestion des effectifs, l'organisation du travail et la prestation des services de santé. Conséquemment, ces changements tendent à bouleverser la nature de la profession, la qualité et les conditions de travail ainsi que le caractère constitutif du climat de travail.

Health and social care in every system and in every country is labour intensive, and must be oriented to people's needs if it is to be effective. It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy.
