La fréquentation des urgences pédiatriques (UP) augmente depuis plusieurs années. Le recours au service des UP s'avère souvent inapproprié. Notre étude a pour but de caractériser les enfants et leurs familles sur le plan socioéconomique et leur parcours de soins afin d'établir un profil de ces populations. Cette étude prospective a été menée dans le service des UP du CHU d'Angers en décembre 2012. 104 dossiers étaient analysés, la traumatologie et les urgences vitales étaient exclues. Résultats: L'âge moyen des enfants était de 4,2 ans, ils étaient accompagnés par leur mère dans 87% des cas. L'âge moyen des parents était de 34,2 ans. 30% des mères étaient sans activité professionnelle. 11% des parents étaient d'origine étrangère. 71% du suivi des enfants était assuré par un médecin généraliste. 40% des parents avaient pris la décision de se rendre aux urgences seuls avec comme raisons citées: la gravité ou la persistance des symptômes (48%) et l'indisponibilité du médecin généraliste (20%). 60% des enfants étaient déjà passés par les UP. 50% des consultations aux UP n'étaient pas justifiées alors que 91% des parents trouvaient la situation urgente. Les consultations étaient plus souvent justifiées quand l'urgence était jugée très importante par les parents (p=0,02). L'origine étrangère des parents était associée à un plus fort taux de consultations non urgentes (p=0,02). L'âge des enfants, la composition du foyer, la couverture sociale et les revenus n'influençaient pas le taux de recours inapproprié aux UP. Sous réserve d'un faible échantillon de population, notre étude ne montre pas une plus grande fréquentation des UP par les foyers de bas niveau socioéconomique.

Introduction : On assiste à un accroissement du nombre de consultations pédiatriques aux urgences, de 3 à 5% par an, pour des motifs considérés comme relevant bien souvent de la médecine générale. Bien que le taux d'admission appropriée augmente significativement lorsque l'enfant est vu par son médecin, celui-ci adresse 25 % de pathologies bénignes. Objectif : Définir les facteurs décisionnels qui ont incité le médecin généraliste à adresser l'enfant aux urgences pédiatriques. L'objectif secondaire est d'évaluer les recours éventuels en amont de la prise en charge hospitalière, en particulier le recours à un référent en pédiatrie. Matériel et mérhode : Il s'agit d'une enquête de pratique, qualitative, destinée aux médecins ayant adressé un enfant aux urgences pédiatriques de Bordeaux. Les médecins ont été recrutés à partir d'un questionnaire accompagnant l'admission de chaque enfant adressé par son médecin généraliste. Puis 31 entretiens semi-directifs téléphoniques ont été secondairement réalisés. Résultats : La gravité et/ou la persistance des symptômes, l'échec de prise en charge en amont, l'âge et l'environnement de l'enfant sont des facteurs identifiés comme ayant influencé les médecins à adresser l'enfant aux urgences. D'autres éléments plus vastes ressortent des entretiens et ont eu un impact sur la prise de décision, tel que l'expérience du médecin, son apprentissage, sa pratique en pédiatrie et sa perception au moment de la prise de décision. En amont, le recours à un référent en pédiatrie dans notre étude fut restreint. Conclusion : La prise de décision amenant un médecin généraliste à adresser aux urgences pédiatriques est complexe et répond à l'influence de multiples facteurs. L'amélioration de la prise en charge globale de l'enfant passe par une meilleure collaboration entre la ville et l'hôpital et entre l'ensemble des acteurs intervenant en pédiatrie. Des propositions ont émergé des entretiens et pourraient fortement contribuer à améliorer ces échanges.
