La douleur aiguë ou chronique des sujets âgés institutionnalisés est un problème fréquent en soins primaires. La prévalence de la douleur chronique en institution est estimée entre 49 et 83%. Par ailleurs 42% seraient atteints de maladie d’Alzheimer ou syndromes apparentés. Chez les patients qui présentent des troubles de la communication verbale et/ou une désorganisation de la pensée, la douleur peut s’exprimer par des symptômes psychologiques et comportementaux de la démence(SPCD). Afin d’optimiser la démarche diagnostique et évaluative de la douleur des outils sont validés et diffusés mais probablement mal ou sous utilisés. De nombreuses études ont montré un sous diagnostic et un sous traitement de la douleur, a fortiori chez les patients atteints de troubles cognitifs. Une étude transversale observationnelle a été menée dans six EHPAD de Basse-Normandie. 109 patients ont été inclus, dont 4 exclus après détection d’un syndrome confusionnel. Les SPCD ont été évalués par la passation de l’inventaire neuro-psychiatrique( NPI), et la douleur par l’Echelle Verbale Simple, complétée en cas de doute ou de négativité des échelles Algoplus et Doloplus. La prévalence de la douleur était de 74.3%. Parmi les 78 patients détectés douloureux, 64 recevaient un traitement antalgique, 61 un traitement psychotrope, et 27 une contention physique régulière. Les SPCD et l’attitude adoptée jusqu’à lors les concernant ont été étudiés. Les spécificités de la population âgée démente impliquent une démarche de soins adaptée. L’apparition ou la majoration d’un ou plusieurs SPCD doit toujours faire rechercher un syndrome douloureux sous-jacent. La sensibilisation et la formation de l’ensemble des équipes soignantes sont indispensables.
