Notre travail traite du parcours professionnalisant des cadres de santé de la médecine des forces. Au regard des récentes transformations sanitaires et militaires (Ambition 2030, guerre en Ukraine, Ségur de la santé, crise sanitaire), le périmètre d'action du cadre de santé des forces a constamment évolué depuis sa mise en place en 2018. C'est dans ce contexte que la politique de haut encadrement militaire paramédical est en germination alors que le nombre de candidats se présentant au concours de cadre de santé diminue chaque année et que des places ouvertes à la sélection militaire sont régulièrement non pourvues. L'ensemble de ces éléments met en exergue un besoin d'adaptation rapide. Par conséquent, dans le cadre de notre travail nous nous interrogeons ainsi : en quoi l'adaptation du parcours professionnalisant des cadres de santé est un enjeu d'attractivité pour la filière managériale au sein de la médecine des forces ? Ce travail nous donne ainsi l'opportunité de nous questionner sur les spécificités du métier de cadre de santé dans la médecine des forces et de la co-construction de son image à travers des parcours professionnalisants adaptés à ses conditions d'exercice afin d'optimiser ses aptitudes aux postes à hautes responsabilités et compenser certaines iniquités. En outre, un important travail de délégation de responsabilité doit être acté afin de rendre le métier plus attractif. Dans le cadre de notre travail de recherche, nous avons effectué une étude quantitative en utilisant des questionnaires dématérialisés afin de mettre en évidence le ressenti et les suggestions des acteurs de terrain comme des potentiels futurs cadres de santé des forces : les infirmiers de la médecine des forces. De ces éléments, des propositions pour agir sont formulées sur des thématiques d'organisation, de ressources humaines et de réglementation dont la mise en œuvre serait souhaitable à court ou à moyen terme.

Health and social care in every system and in every country is labour intensive, and must be oriented to people's needs if it is to be effective. It is now widely recognized that human resources for health (HRH) are a key enabler for the attainment of universal health coverage, and for the achievement of Sustainable Development Goal (SDG) 3 - Ensure healthy lives and promote well-being for all at all ages. As is stressed in the Global Strategy on Human Resources for Health: Workforce 2030, there can be no viable national, or global, health system without an effective health workforce. The Global Strategy, adopted at the Sixty-ninth World Health Assembly in May 2016, challenges the erroneous narrative of health workers as a unit of cost in the production of health. The evidence instead presents an intersectoral agenda on the pre-condition of equitable access to health workers in the attainment of universal health coverage, along with a dynamic labour market understanding of the substantive impact on education, employment, jobs and innovation in the health and social care economy. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
