Le vieillissement de la population s'accompagne de problèmes de dépendance et de maintien à domicile dont un des acteurs essentiel est l'aidant naturel. Ce travail a été mené en Seine Saint Denis au travers d'une enquête quantitative. 129 questionnaires ont été diffusés et ciblaient des médecins généralistes (MG). L'objectif de l'étude était de déterminer le rôle et les limites du MG dans la prise en charge des aidants. Les principales missions identifiées par les MG étaient le soutien moral, la coordination des soins et l'organisation des solutions de répit. La majorité des médecins ne pratiquaient pas de suivi des aidants. Des limites ont été soulevées : défaut dans la formation initiale, problématique des visites à domicile, manque de lisibilité et répartition inégale de l'offre de répit sur le territoire. Le MG aura à l'avenir ces missions de prise en charge globale du sujet âgé, pour en garantir le succès il lui faudra savoir anticiper l'épuisement de l'aidant.

Introduction : La maladie d'Alzheimer (MA) est la plus fréquente des démences du sujet âgé. En France, elle touche environ 800000 personnes âgées de 75 ans et plus. Elle est considérée comme la principale cause de dépendance lourde de la personne âgée et représente le premier motif d'institutionnalisation. Notre objectif est d'explorer le vécu de l'aidant principal avant et après l'institutionnalisation pour mieux comprendre sa situation, ses difficultés et préciser ainsi le rôle que le médecin généraliste peut tenir pour mieux l'aider. Matériel et méthodes : Étude qualitative sur 10 entretiens semi-dirigés qui ont été menés durant 4 mois auprès de 11 aidants familiaux principaux dont le proche a été diagnostiqué Alzheimer (ou démence apparentée) et institutionnalisé de façon définitive dans une unité du CH de Saint-Nazaire. Résultats : Le temps moyen entre le diagnostic et l'institutionnalisation a été de 4 ans. Les troubles du comportement, la nécessité d'une mobilisation de chaque instant, la peur des accidents domestiques, les fugues et la perte d'autonomie ont été les principales difficultés rencontrées par les aidants au domicile. La plupart s'informaient d'eux-mêmes pour avoir recours à des aides extérieures. Le choix de l'aidant était implicite et la majorité était des femmes, retraitées, proches géographiquement et disponibles sentimentalement. L’institutionnalisation s'est faite, pour 8 cas sur 10, dans la continuité de séjours dans les structures de répit (accueil de jour et hébergement temporaire) et souvent à la suite d'un événement déclenchant. Le ressenti après l'institutionnalisation est différent selon les cas, le plus douloureux a été le travail de «deuil blanc». Le médecin traitant a eu un rôle de coordinateur des soins et de soutien pour une minorité d’aidants. Conclusion : Afin d'améliorer la prise en charge de la MA, il semble nécessaire de repenser l'accompagnement du malade et de son aidant aussi bien avant qu'après l'institutionnalisation. Dans ce sens, le rôle du médecin traitant est primordial et doit être renforcé. De plus, il serait intéressant d'analyser l'apport sur le territoire français des Maisons pour l'Autonomie et l'Intégration du malade d'Alzheimer (MAIA).

But the history of Christmas cannot be summed up so easily as the commercialization of a sacred day. As Penne Restad reveals in this marvelous new book, it has always been an ambiguous meld of sacred thoughts and worldly actions-- as well as a fascinating reflection of our changing society. In Christmas in America, Restad brilliantly captures the rise and transformation of our most universal national holiday. In colonial times, it was celebrated either as an utterly solemn or a wildly social event--if it was celebrated at all. Virginians hunted, danced, and feasted. City dwellers flooded the streets in raucous demonstrations. Puritan New Englanders denounced the whole affair. Restad shows that as times changed, Christmas changed--and grew in popularity. In the early 1800s, New York served as an epicenter of the newly emerging holiday, drawing on its roots as a Dutch colony (St. Nicholas was particularly popular in the Netherlands, even after the Reformation), and aided by such men as Washington Irving. In 1822, another New Yorker named Clement Clarke Moore penned a poem now known as "'Twas the Night Before Christmas," virtually inventing the modern Santa Claus. Well-to-do townspeople displayed a German novelty, the decorated fir tree, in their parlors; an enterprising printer discovered the money to be made from Christmas cards; and a hodgepodge of year-end celebrations began to coalesce around December 25 and the figure of Santa. The homecoming significance of the holiday increased with the Civil War, and by the end of the nineteenth century a full- fledged national holiday had materialized, forged out of borrowed and invented custom alike, and driven by a passion for gift-giving. In the twentieth century, Christmas seeped into every niche of our conscious and unconscious lives to become a festival of epic proportions. Indeed, Restad carries the story through to our own time, unwrapping the messages hidden inside countless movies, books, and television shows, revealing the inescapable presence--and ambiguous meaning--of Christmas in contemporary culture.
