Une question se pose alors : Quelles sont les difficultés rencontrées pour la mise en place de ces structures innovantes ? La description et l'observation pendant ces deux dernières années du projet de Neufchâtel en Bray, révèlent des problématiques de création d'une M.S.P. Les professionnels de santé présentent des réticences individuelles, des difficultés d'organisation et des obstacles financiers pour la concrétisation du projet. Dans le cadre d'un regroupement médical, les patients s'inquiètent de la disponibilité de leur médecin traitant, du respect de leur liberté du choix de médecin, et surtout, de la préservation de la relation médecin-malade. Dans le cadre d'un exercice pluridisciplinaire, les craintes reposent sur la confidentialité des données médicales. Les institutions et les élus politiques analysent les inégalités géographiques de l'offre de soins, pour proposer des solutions adaptées. Les aides au financement des M.S.P. se mettent en place pour dynamiser les zones sous dotées en médecins, dans une politique plus large d'aménagement du territoire. Les difficultés sont d'autant plus nombreuses que la réussite des projets repose sur la convergence des motivations et des contraintes de tous les acteurs du système de soins, dont les problématiques s'entrecroisent.
