Le caractère dégénératif de la maladie rend inadaptée une intervention centrée sur la restauration des fonctions perturbées. Une meilleure intervention serait une adaptation de l'entourage aux troubles des patients afin de maintenir au mieux leurs capacités de communication. Dans cette perspective, l'équipe canadienne ESPACE a développé une approche basée sur la prise en compte de facteurs personnels et environnementaux afin de former l'entourage à la communication. Cette approche « sociale» nous a ,conduits à évaluer la communication entre quatre patients Alzheimer institutionnalisés et leur entourage familial, puis à former les aidants en leur apportant des stratégies de communication. Pour cela, nous avons construit un profil spécifique de chaque résident, servant de base à l'accompagnement et à l'adaptation des familles. Notre expérimentation montre que ce type d'intervention est bénéfique puisque les aidants deviennent plus compétents : ils cernent les difficultés et besoins spécifiques de leur proche, utilisent plus de moyens de communication efficaces, et éprouvent moins d'effort lors des interactions. L'accompagnement de l'entourage doit s'étendre au-delà des techniques de communication. Un éclairage théorique, une attitude empathique sont également utiles aux interactions et par conséquent au bien-être des aidants et de leur proche.
