The Joint External Evaluation (JEE) team acknowledges Nigeria for volunteering for a second JEE after holding its first in 2017 and an internal mid-term JEE in 2019. This JEE uses the third edition of the JEE tool. This revised third edition of the tool incorporates relevant lessons from the ongoing COVID-19 pandemic and other public health emergencies. Nigeria’s broad efforts to prepare for and respond to emergencies have clearly facilitated its whole-of government response to the COVID-19 pandemic. The unprecedented nature, magnitude and wide societal impact of the pandemic has led to improvisation and innovation. As a result, emergency risk management stakeholders in Nigeria have been keen to combine the many lessons of the pandemic response – consolidated through intra-action reviews – and to address gaps in IHR core capacities identified through the JEE. The JEE team commends Nigeria for its efforts to monitor and improve health security with openness, transparency and goodwill.

The United Republic of Tanzania–Zanzibar completed its first Joint External Evaluation (JEE) from 22 to 28 April 2017. The present review, which took place just over six years later (21–25 August 2023), was the second JEE for Zanzibar. It was also the world’s 133rd JEE (in 122 countries) and the 55th in the WHO African Region. The Joint External Evaluation (JEE) team expresses its appreciation to Zanzibar for its commitment to health security and for volunteering for a JEE for the second time. This evaluation was conducted using the JEE tool version 3. This revised third edition of the tool incorporates lessons learned from the COVID-19 pandemic and other public health emergencies. It was realized that the level of capacity that was previously thought to be sufficient was not. Consequently, the tool was substantively reworked, and the standard has been increased throughout. This makes it particularly important to emphasize that direct comparison of scores alone is both invalid and contraindicated. This applies to scores between countries and also scores resulting from the use of different versions of the JEE tool.

Public health emergencies continually reinforce the need for an integrated approach to health systems strengthening, underpinned by a public health approach, helping to build health systems resilience. As part of a collaboration between the USAID Office of the Health Systems and the WHO Integrated Health Services Department, a strategic meeting was convened on 14 April 2021. This had wide-ranging representation which included WHO Deputy Director-General, Executive leadership from USAID, IANPHI and WFPHA. The overarching aim was to examine this topic together and identify actionable steps to build an integrated approach to health system strengthening that brings together health security, humanitarian, disease-specific and life-course-specific programmes. The specific objectives of the meeting were to: facilitate strategic discussions on prevailing practices in health systems resilience for health protection and high-quality health services, to determine actionable next steps for global guidance; inform policy options for WHO, USAID and partners to maximize joint country support for health systems resilience; and identify immediate priorities for next steps following the meeting. To inform and support the proceedings of this meeting, complementary desk reviews were conducted to synthesise evidence, and inform the discussions and way forward. The International Health Regulations (IHR) establish risk communicationâ€"the real-time exchange of information, advice, and opinions between experts or officials and people who face a threat to their survival, health, and economic or social well-beingâ€"as a core capacity that World Health Organization member states must fulfill to strengthen the fight against these threats. Despite global recognition of the importance of complying with IHR, 67 percent of signatory countries report themselves as not compliant. By investing in communication capacity, public health and government officials and civil society organizations facing health crises would be prepared to provide advice, information, and reassurance to the public as well as to rapidly develop messages and community engagement activities that are coordinated and take into account social and behavioral dynamics among all sectors. The Framework for a Public Health Emergency Operations Centre (PHEOC framework) document is intended to be used by practitioners of public health; health policy makers; and authorities and agencies responsible for managing emergencies, incidents, or events where the health of populations is at risk. This document provides high-level methodical guidance for designing, developing, and strengthening of public health emergency operations centers. This interim document outlines the key concepts and essential requirements for developing and managing a public health EOC (PHEOC). The overall approach is generic and based on widely acknowledged elements of all-hazards emergency management. It provides an outline for developing and managing a PHEOC to achieve a goal-oriented response to public health emergencies and unity of effort among response agencies. The document will be revised as necessary. Practical guidance on specific aspects of the PHEOC framework will be developed and published separately. A public health emergency is here defined as an occurrence, or imminent threat, of an illness or health condition that poses a substantial risk of a significant number of human fatalities, injuries or permanent or long-term disability. Public health emergencies can result from a wide range of hazards and complex emergencies. Experience has shown that timely implementation of an EOC provides an essential platform for the effective management of public health emergencies. Public health emergencies involve increased incidence of illness, injury and/or death and require special measures to address increased morbidity, mortality and interruption of essential health services. For such emergencies, a multi-agency, multi-jurisdictional response is often required, working with the national disaster management organization. When normal resources and capacities are exceeded, support from outside the affected areas will also be required. External assistance could include national, cross-border, regional or international resources.
