Les Anti-inflammatoires non stéroïdiens sont parmi les médicaments les plus prescrits dans le monde avec une utilisation s’étendant bien au-delà des seules maladies inflammatoires. Ils viennent en effet au premier rang de la consommation médicamenteuse à visée analgésique. En novembre 2005, la Haute Autorité de Santé a publié des recommandations sur la prise en charge de la douleur postopératoire en chirurgie buccale. Celles-ci suggèrent l’utilisation d’AINS lors de la prise en charge de douleurs d’intensité modérée à sévère. De plus, les données de la littérature mettent en évidence l’utilisation de molécules et de posologies parfois très différentes selon les pays et les spécialités. Au vu de ces recommandations récentes, plusieurs questions restent encore à résoudre: les AINS possèdent-ils une activité anti-inflammatoire associée à leur activité antalgique? Existe-t-il des différences significatives en termes d’efficacité et d’effets indésirables entre toutes les molécules d’AINS ? Les effets indésirables décrits avec ces médicaments dans une indication rhumatologique peuvent-ils être extrapolés à leur utilisation en chirurgie buccale? Existe-t-il un lien de causalité pertinent entre la prise d’AINS en chirurgie buccale et le développement potentiel d’une infection locorégionale? Ce travail de thèse propose de répondre à ces différentes questions à travers une analyse de la littérature.

La prise en charge de la douleur postopératoire est une préoccupation majeure dans le domaine de la chirurgie buccale. La prescription d’anti-inflammatoires non stéroïdiens ou d’antalgiques n’est pas sans effets indésirables. Le traitement dit « laser assisté)) a été démontré dans différentes études, capable d’offrir certaines possibilités analgésiques. Les effets antalgiques du laser les plus fréquemment rapportés concernent l’action sur les endorphines, sur la stabilisation de la membrane cellulaire ou encore l’action directe de la LLLT (Low Level Laser Therapy) sur la sensibilisation des nocicepteurs. Toutefois, au cours de ces dix dernières années, les auteurs semblent de plus en plus en faveur d’une action antalgique principalement voire exclusivement anti-inflammatoire du laser. La LLLT permettrait de réguler la production ainsi que l’expression de nombreuses molécules inflammatoires. Cliniquement, un nombre encore trop faible d’études met en évidence l’intérêt du laser dans le contrôle des douleurs postopératoires. Néanmoins, les résultats récemment obtenus semblent tendre vers une diminution de la douleur chez les patients ayant reçus une dose de laser en postopératoire. La problématique porte sur le fait que dans la plupart des études cliniques réalisées, le choix des lasers et des caractéristiques de l’irradiation ont été fait arbitrairement

Il semble nécessaire de s’interroger sur la prise en charge de la douleur et de l’inflammation en pratique odontostomatologique, en effet les AINS sont trop souvent prescrits sans avoir de réelle utilité lorsqu’il s’agit d’un processus inflammatoire. La posologie autorisée de AINS utilisés en odontologie, en France (1200 mg/j) est loin d’atteindre le seul d’activité anti-inflammatoire. Le but de cette thèse est de montrer que les glococorticoïdes, anti-inflammatoire (AIS) à défaut d’avoir une activité antalgique présentent une action anti-inflammatoire bien supérieure aux AINS et ont donc un intérêt réel lors de chirurgie buccale ou bien même ne dermatologie buccale, leurs indications en odontostomatologie sont nombreuses. Les effets indésirables et les contre-indications des AIS sont limités d’autant plus qu’ils sont utilisés en cure courte dans notre domaine. Les AINS présentent également leurs propres indications mais il s’agit avant tout d’antalgiques dont l’activité est supérieure à celle du paracétamol, leur utilité est importante en odontostomatologie pour gérer la douleur à condition de tenir en compte de l’ensemble de leurs effets indésirables et de leurs contre-indications qui paraissant nombreux.

It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.
