The Military Health System (MHS) finds itself operating in a competitive market. TRICARE, the military managed care system, allows family members to seek care outside the uniformed network. In this competitive environment, it is necessary to deliver health care in such a way that meets or exceeds patients expectations and perceptions. Doing so not only ensures the existence of a robust MHS, but it is simply the right thing to do. As such, the staff at Winn Army Community Hospital (WACH) takes patient satisfaction very seriously. This project monitored the maturation and influence of a Process Action Team (PAT). The Hospital Commander chartered the team in order to find ways in which the Department of Emergency Medicine (DEM) staff could improve patient throughput for the most serious injuries and illnesses presenting to the department and, by so doing, increase patient satisfaction. The project followed the specific steps the team took to improve throughput, based on the variables it identified as having an influence on overall operations. Analysis of variance (ANOVA) compared the average time 1200 patients waited to see a physician in the DEM for the six months prior to and after the teams first meeting. This study investigates hospital strategies used to inform patients of anticipated wait times in the Emergency Department (ED) and the association of such strategies with patient throughput efficiency. When hospitals are inefficient in ED patient throughput, conditions of overcrowding often occur. Overcrowding of the ED is a problem for many hospitals. When such conditions exist, there is evidence in the literature that it leads to lower patient satisfaction, care quality, and financial position of the hospital. Therefore, improving patient throughput efficiency is a priority for hospitals. To improve patient throughput efficiency, hospitals use a variety of strategies. This study focuses on two wait time communication strategies of posting ED wait times on the hospital website and the use of ED reservation systems. Through application of Rational Choice Theory, it is expected that patients who are informed of anticipated wait times will make rational decisions related to visiting a potentially overcrowded ED. Thus, engaged patients make decisions that can contribute to improved ED efficiency. The question of this research study is "Do ED wait time communication strategies improve patient throughput efficiency?" This study sample includes acute care hospitals in Florida and the ED throughput efficiency metrics from the Center for Medicare and Medicaid Services (CMS) Hospital Compare data sets to contrast the performance of hospitals that use the study strategies. The results of the study indicate that posting ED wait times on the hospital website has a statistically significant association with ED efficiency. However, the use of reservation systems does not have a statistically significant association with ED efficiency. Further, the control variables of hospital licensed bed size, metropolitan location, percent of population without health insurance, and percent of population Medicaid eligible have a statistically significant association with ED efficiency. Consistent with expected behavior related to Rational Choice Theory, this study supports that informed patients will make logical decisions related to if and when to visit an ED for care and therefore contribute to improved throughput efficiency.

The purpose of this project was to evaluate the patient experience in the emergency department (ED) and in the inpatient setting while correlating increased throughput and patient outcomes at a suburban Acute Care facility in Ohio. The culture in the organization has lacked accountability and ownership of the patients. The ED admitted length of stay (ALOS) was 358 minutes in the beginning of 2013. For the first time in the organization's history, the ED ALOS is now typically less than the recommended benchmark of 300 minutes. A report of findings among ED's surveyed showed the ALOS best practice is 244 minutes with a median length of stay of 309 minutes (Premier, 2006, slide 13). Throughout its recent history, the organization has failed to have a service-oriented approach to patient care. Quality improvement was identified and implemented through a hospital-wide Kaizen event focused on throughput of the admitted patient (Appendix A). According to King (2010), the Japanese words Kai and Zen literally means "to change" and "for the better", and it has come to symbolize continuous improvement (slide 5). Teams of individuals ranging from physicians to transporters spent one week of work time focused on the process mapping of the current state and future state of ED throughput. The use of information technology (IT) in the process improvement was integral to performance improvement, patient safety, and consistent ED ALOS less than 300 minutes.
