Le psoriasis constitue 5% des affections dermatologiques pédiatriques. Il se caractérise par une production massive des kératinocytes et par une inflammation de la peau qui est une conséquence de la prolifération des lymphocytes T. Le psoriasis pourrait donc être une maladie du système immunitaire dont la peau serait le révélateur. La diversité des formes cliniques rencontrées chez l'enfant laisse suspecter qu'il est sans doute préférable de parler "des psoriasis" plutôt qu'évoquer une maladie univoque. Le traitement topique, indiqué en première intention, est souvent suffisant pour permettre une prise en charge correcte de la majorité des cas. On utilise en premier lieu, la corticothérapie locale et les dérivés de la vitamine D. En cas d'inefficacité ou de récidive rapide et sévère, la photothérapie TL-01 est indiquée en respectant les contre-indications, l'âge de l'enfant et en limitant le nombre de séances. Les traitements systémiques sont ensuite proposés avec en premier lieu l'acitrétine. En cas d'échec, la ciclosporine et le méthotrexate sont alors discutés en suivant de façon très régulière l'enfant. De plus en plus, l'étanercept est utilisé précocement dans l'arsenal thérapeutique car il a prouvé depuis quelques années son efficacité et sa bonne tolérance dans le psoriasis de l'enfant
