La fièvre constitue ainsi un point d'appel important amenant une consultation aux urgences et la mise en place d'une antibiothérapie intraveineuse à large spectre. L'objectif principal de notre étude était de mesurer la prévalence des infections bactériennes des enfants drépanocytaires consultant aux urgences pédiatriques du CHU de Nantes. L'objectif secondaire était d'évaluer les facteurs de risque de ces infections en vue de construire un algorithme d'aide à la décision. Il a été constaté 17 infections bactériennes (13.2 % [IC 95% 7,3-19,1]) dans cette population de patients, parmi lesquelles 3 infections bactériennes invasives (2.3 % [IC 95% 0-4,9]) et 14 infections bactériennes sévères (10,9% [IC 95% 5,5-16,3]). La CRP et la PCT étaient significativement supérieures dans les cas d'infections bactériennes par rapport aux épisodes de fièvre sans infection bactérienne. L'algorithme d'aide à la décision que nous avons proposé devra être validé sur une autre population puis évalué à l'occasion d'une étude clinique prospective et multicentrique.
