Book Description
Federal Fumbles: Ways the Federal Government Dropped the Ball Vol. 4 2019 includes some pretty significant changes to the book this year. Since the first edition in 2015, Federal Fumbles has focused on government waste, which has generally meant a waste of your tax dollars. This year, Federal Fumbles takes a different approach to "waste." The focus is not only on waste of your tax dollars but also on waste in good government policy that prevents the federal government from working for you-or more recently, from working at all due to the longest government shutdown in American history. Our national debt continues to increase and our budget process remains broken. We have officially reached a staggering $22 trillion in national debt. Even with a booming economy, Congress cannot seem to curb annual deficits or fund our government on time. Highlighting issues in previous Fumbles volumes ("Doggy Hamlet," anyone?) only goes so far. This year Federal Fumbles highlights substantial reforms to government programs that not only save tax dollars but provide more transparent and efficient programs for you and your family. We need relief from our national debt and clarity in other complex issues currently facing our nation, but we also need to promote big ideas for big solutions for big reform.Too often, complicated issues like our federal budget process and immigration are clouded with misinformation, convoluted figures, and partisan angles. This year's edition of Fumbles works to break those big issues down for you. Every issue highlighted in this book comes with a solution. In order to fix many of government's fumbles, we must be transparent and real about facts instead of driving home a political agenda. These are exceptionally difficult times in government. Let's use Fumbles to help resolve some of the most pressing and costly problems facing our country while protecting your tax dollars. The goal for this book is to make it a resource for taxpayers and the entire federal government. Congress, federal agencies, and the White House should all take a look at the content covered in this book and the solutions presented. Washington works for the American people and must be held accountable by the American people. Fumbles outlines a set of ideas about ways we can work together to make that happen.

Book Description
Many of the photos were removed due to copyright restrictions. Welcome to the first annual release of the Federal Fumbles report! Our national debt is careening toward $19 trillion (yes, that is a 19 followed by 12 zeros), and federal regulations are expanding at a record pace. Meanwhile families struggle to get home loans, and small businesses struggle to make ends meet. States are constantly handed unfunded mandates and executive fiats that they are forced to implement with minimal direction and no way to pay for them. I present this report as a demonstration of ways we can cut back on wasteful federal spending and burdensome regulations to help families, small businesses, and our economy begin to get out from under the weight of federal stagnation. Cited here are not only prime examples of wasteful spending, but also federal departments or agencies that regulate outside the scope of the federal government's constitutional role. I firmly believe my staff and I have the obligation to solve the troubles of our nation, not just complain, which is why for every problem identified, you will also find a recommended solution. There is a way to eliminate wasteful, ineffective, or duplicative program spending; develop oversight methods to prevent future waste; and find ways to get us back on track.

Book Description
Malcolm Gladwell, host of the podcast Revisionist History and author of the #1 New York Times bestseller Outliers, offers a powerful examination of our interactions with strangers and why they often go wrong—now with a new afterword by the author. A Best Book of the Year: The Financial Times, Bloomberg, Chicago Tribune, and Detroit Free Press How did Fidel Castro fool the CIA for a generation? Why did Neville Chamberlain think he could trust Adolf Hitler? Why are campus sexual assaults on the rise? Do television sitcoms teach us something about the way we relate to one another that isn’t true? Talking to Strangers is a classically Gladwellian intellectual adventure, a challenging and controversial excursion through history, psychology, and scandals taken straight from the news. He revisits the deceptions of Bernie Madoff, the trial of Amanda Knox, the suicide of Sylvia Plath, the Jerry Sandusky pedophilia scandal at Penn State University, and the death of Sandra Bland—throwing our understanding of these and other stories into doubt. Something is very wrong, Gladwell argues, with the tools and strategies we use to make sense of people we don’t know. And because we don’t know how to talk to strangers, we are inviting conflict and misunderstanding in ways that have a profound effect on our lives and our world. In his first book since his #1 bestseller David and Goliath, Malcolm Gladwell has written a gripping guidebook for troubled times.

Book Description
As occupational therapy celebrates its centennial in 2017, attention returns to the profession's founding belief in the value of therapeutic occupations as a way to remediate illness and maintain health. The founders emphasized the importance of establishing a therapeutic relationship with each client and designing an intervention plan based on the knowledge about a client's context and environment, values, goals, and needs. Using today's lexicon, the profession's founders proposed a vision for the profession that was occupation based, client centered, and evidence based--the vision articulated in the third edition of the Occupational Therapy Practice Framework: Domain and Process. The Framework is a must-have official document from the American Occupational Therapy Association. Intended for occupational therapy practitioners and students, other health care professionals, educators, researchers, payers, and consumers, the Framework summarizes the interrelated constructs that describe occupational therapy practice. In addition to the creation of a new preface to set the tone for the work, this new edition includes the following highlights: a redefinition of the overarching statement describing occupational therapy's domain; a new definition of clients that includes persons, groups, and populations; further delineation of the profession's relationship to organizations; inclusion of activity demands as part of the process; and even more up-to-date analysis and guidance for today's occupational therapy practitioners. Achieving health, well-being, and participation in life through engagement in occupation is the overarching statement that describes the domain and process of occupational therapy in the fullest sense. The Framework can provide the structure and guidance that practitioners can use to meet this important goal.

Book Description
. Renewal of Life by Transmission. The most notable distinction between living and inanimate things is that the former maintain themselves by renewal. A stone when struck resists. If its resistance is greater than the force of the blow struck, it remains outwardly unchanged. Otherwise, it is shattered into smaller bits. Never does the stone attempt to react in such a way that it may maintain itself against the blow, much less so as to render the blow a contributing factor to its own continued action. While the living thing may easily be crushed by superior force, it none the less tries to turn the energies which act upon it into means of its own further existence. If it cannot do so, it does not just split into smaller pieces (at least in the higher forms of life), but loses its identity as a living thing. As long as it endures, it struggles to use surrounding energies in its own behalf. It uses light, air, moisture, and the material of soil. To say that it uses them is to say that it turns them into means of its own conservation. As long as it is growing, the energy it expends in thus turning the environment to account is more than compensated for by the return it gets: it grows. Understanding the word "control" in this sense, it may be said that a living being is one that subjugates and controls for its own continued activity the energies that would otherwise use it up. Life is a self-renewing process through action upon the environment.

Book Description
Getting the right diagnosis is a key aspect of health care - it provides an explanation of a patient's health problem and informs subsequent health care decisions. The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Yet, he was largely held to blame for the Confederacy's defeat at Gettysburg. General James Longstreet sheds new light on the controversial commander and the man Robert E. Lee called “my old war horse.”
