A study of safety reports was conducted to examine the hypothesis that fatigue and associated performance decrements occur in air transport operations, and that these are associated with some combination of factors: circadian desynchronosis, duty time; pre-duty activity; sleep; work scheduling; workload; and environmental deprivation. The findings are based on a selected sample of reported incidents in which the reporter associated fatigue with the occurrence. In comparing the fatigue reports with a control set, significant performance decrements were found to exist related to time-of-day, awareness and attention to duty, less significantly, final phases of flights. The majority of the fatigue incidents involved such unsafe events as altitude deviations, takeoffs and landing without clearance, and the like. Simulation studies of flight crew performance should be utilized to determine the degree of fatigue induced by demanding schedules and to delineate more precisely the factors responsible for performance decrements in flight and to test solutions proposed to resolve problems induced by fatigue and desynchronosis. Fatigue encountered in airlifts to foreign military operations can be severe enough to pose a flight safety hazard. This study is an attempt to document to what extent fatigue (and time zone changes of five & six hours) can impact on aircrew performance during routine re-supply missions to former Yugoslavia. In the first phase of the study, aircrew sleep hygiene was assessed immediately prior to & throughout ten missions to Bosnia. Aircrew psychomotor performance was also assessed during flight. Phase two involved a laboratory-based fatigue countermeasure study in which melatonin and zopiclone were evaluated as facilitators of early circadian sleep. In this phase, residual effects of these medications on aircrew performance after sleep were also studied. The final phase was an operational evaluation of the lab-proven countermeasures in 70 missions to Bosnia. The performance of aircrew using the medications is compared to performance using placebos. At the close of this phase, a laboratory study also compared a new short-acting sleep medication, zaleplon, against two other sleep medications (zopiclone and temazepam) and melatonin in order to quantify the depth of their respective impacts on psychomotor performance and the post-ingestion time required for return to normal performance.

Nearly everyone experiences fatigue, but some professions--such as aviation, medicine and the military--demand alert, precise, rapid, and well-informed decision making and communication with little margin for error. The potential for fatigue to negatively affect human performance is well established. Concern about this potential in the aviation context extends back decades, with both airlines and pilots agreeing that fatigue is a safety concern. A more recent consideration is whether and how pilot commuting, conducted in a pilot's off-duty time, may affect fatigue during flight duty.
