The number of women living with HIV/AIDS is increasing steadily. Very few research studies have been done to understand the experiences of older women living with the disease. The purpose of my study was to understand the challenges faced by midlife women living and aging with HIV/AIDS through the progression of their illness. Exploratory case studies were developed of three women between the ages of 50-55 years of age. These women are only in their early midlife, but because of their risky life style factors, HIV treatment side effects and other comorbidities are dealing with disabilities, emotional impacts, difficult lifestyle adjustments, and decline in functional capacities. They are unable to use services provided for older adults and are excluded because of construction of age categories in providing services. We need to adopt a life course approach in providing community and healthcare services for these women, as their roles and challenges change in living and aging with HIV. The elements of the Chronic Care Model were applied to suggest how the quality of life of these women could be enhanced as their needs change throughout their lives.

Black women, ages 45 and older, are disproportionately affected by the HIV/AIDS epidemic. In 2001, Black women comprised 11% of all older women in the United States; however, they accounted for more than 50% of AIDS cases and more than 65% of HIV cases (Winningham et al., 2004). The literature review reflected little research specifically looking at the reason(s) for the increase in HIV in Black women, ages 45 to 60. Furthermore, few research studies focused on HIV patient health beliefs and their experiences in living with HIV. This study was conducted with an ethnography research method coupled with the American Black feminist theory. The primary purpose of this qualitative study was to examine the reason(s) why the HIV rate is growing at an alarming pace for Black women ages 45 to 60 years old. The research instruments consisted of a demographic form and a six-question interview guide that formed the basis of 60–90 minute interviews meeting the research criteria. Demographic information was collected from a self-administered survey that included age, employment status, income, education level, date of infection, source of infection, date treatment started, whether the participant was currently in treatment or not in treatment, and the reasons for that treatment decision. This was a non-random sample of convenience. This researcher chose not to conduct a random sample because of the parameters of the inclusion and exclusion criteria. The participants consisted of 22 HIV-positive midlife Black women between the ages of 45 and 60 years old who self-selected to participate in the research study; however, two of the participants declined to have their interviews included in the results. Therefore, 20 participant-encounters were included. IRB approval was obtained from the University of San Diego. The participants signed an informed consent that insured their confidentially. The researcher analyzed the qualitative information by hand coding the data because the ethnic, feminist, and culture elements might lead to misinterpretation if qualitative-analysis software was used. Black women, ages 45 to 60 years offered significant reasons why they were infected with the HIV virus (e.g., lack of education about HIV, media influences, aging, unprotected sex, the onset of drug use after age 40). The findings from this study remain pertinent in all nursing research and advance nursing's utilizing of the American Black feminist theory. Finally, the findings may also facilitate further research into the reason(s) for the increase in HIV infections in Black midlife women. In addition, this study may further the body of research which leads to changes in the delivery and policies of health care.

At a time when lesbian, gay, bisexual, and transgender individuals-often referred to under the umbrella acronym LGBT-are becoming more visible in society and more socially acknowledged, clinicians and researchers are faced with incomplete information about their health status. While LGBT populations often are combined as a single entity for research and advocacy purposes, each is a distinct population group with its own specific health needs. Furthermore, the experiences of LGBT individuals are not uniform and are shaped by factors of race, ethnicity, socioeconomic status, geographical location, and age, any of which can have an effect on health-related concerns and needs. The Health of Lesbian, Gay, Bisexual, and Transgender People assesses the state of science on the health status of LGBT populations, identifies research gaps and opportunities, and outlines a research agenda for the National Institute of Health. The report examines the health status of these populations in three life stages: childhood and adolescence, early/middle adulthood, and later adulthood. Despite decades of attention on building a global HIV research and programming agenda, HIV in older populations has generally been neglected until recently. This new book focuses on HIV and aging in the context of ageism with regard to prevention, treatment guidelines, funding, and the engagement of communities and health and social service organizations. The lack of perceived HIV risk in late adulthood among older people themselves, as well on the part of providers and society in general, has led to a lack of investment in education, testing, and programmatic responses. Ageism perpetuates the invisibility of older adults and, in turn, renders current medical and social service systems unprepared to respond to patients’ needs. While ageism may lead to some advantages – discounts for services, for example – it is the negative aspects that must be addressed when determining the appropriate community-level response to the epidemic.

Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

In sub-Saharan Africa, older people make up a relatively small fraction of the total population and are supported primarily by family and other kinship networks. They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.
