Le développement d'un nouveau médicament est un processus long, coûteux et toujours risqué pour un laboratoire. Après des phases de recherche fondamentale et de développement non-clinique et clinique d'un candidat médicament, le laboratoire va élaborer un dossier d'enregistrement à soumettre aux Agences de santé, pour obtenir une autorisation demise sur le marché. C'est à ce moment là que les Agences de santé vont évaluer le rapport bénéfice/risque du médicament, en se basant sur les résultats d'efficacité et de sécurité des études cliniques. Un médicament est une substance active, qui interagit avec l'organisme et qui soulage, guérit ou augmente les défenses de l'organisme, et apporte un bénéfice santé indéniable mais son utilisation peut présenter des risques. Le rapport bénéfice/risque d'un médicament est difficile à évaluer car de nombreux aspects sont à prendre en compte. En 2009, l'Agence Européenne du Médicament a débuté un projet sur trois ans sur la méthodologie de l'évaluation du rapport bénéfice/risque, dans le but d'harmoniser les méthodes au sein de l'Europe. Ces approches sont très mathématiques pour la plupart, mais doivent aussi prendre en compte le bien-être et les conditions de vie du patient. En effet, en fonction de la maladie à traiter, les patients pourront être plus ou moins prêts à accepter les risques d'un traitement.Le principal objectif de cette thèse est de comprendre comment le rapport bénéfice/risque d'un médicament est évalué en Europe. Pour aider à la réflexion, l'étude d'un médicament contre la sclérose en plaques, Gilenya® (fingolimod),ayant eu des effets indésirables graves après sa mise sur le marché, nous permettra de comprendre comment les Agences de santé peuvent conclure à un rapport bénéfice/risque positif pour un médicament.
