The VIth World Symposium on Cardiac Pacing in Montreal 1979 opened with a course, meant to be an introduction for newcomers and an updating re fresher and link between the various fields of knowledge needed by experienced persons for cardiac pacing. Invited guest lecturers were selected for their world recognized expertise in the individual subjects. This book is a collection of the various presentations on historical, clinical, electrophysiological and technical aspects of cardiac pacing. Together they cover the fundamentals of cardiac stimulation. We hope that this book may become an introductory guide to the field of cardiac pacing and that it may contribute to a better understanding of the pacemaker system and a better treatment of the pacemaker patient. Health spending continues to grow faster than the economy in most OECD countries. In 2010, the OECD published a study of strategies to increase value for money in health care, in which pay for performance (P4P) was identified as an innovative tool to improve health system efficiency in several OECD countries. However, evidence that P4P increases value for money, boosts quality of processes in health care, or improves health outcomes is limited.This book explores the many questions surrounding P4P such as whether the potential power of P4P has been over-sold, or whether the disappointing results to date are more likely rooted in problems of design and implementation or inadequate monitoring and evaluation. The book also examines the supporting systems and process, in addition to incentives, that are necessary for P4P to improve provider performance and to drive and sustain improvement. This study provides the most comprehensive and detailed analysis available on factors behind the decline in maternal mortality in Malaysia and Sri Lanka in the past 50 to 60 years and the magnitude of health system expenditures on maternal health. The main findings are that a modest investment in maternal health services, combined with other poverty reduction measures leads to a fairly rapid decline in the maternal mortality ratio (MMR), defined as the number of maternal deaths per 100,000 live births. The strategies of Malaysia and Sri Lanka changed over time, from an initial emphasis on expanding the provision of services, especially in underserved areas, to increasing utilization and, finally, to emphasizing the improvement of quality. Removing financial barriers to maternal care for clients was an important step in both countries. Professional midwives constitute the backbone of maternal care in Malaysia and Sri Lanka. The MMR reduction in developing countries is feasible with modest public expenditures when appropriate policies are adopted, focused wisely, and adapted incrementally in response to environmental conditions and systems capacity. Agroforestry has come of age during the past three decades. The age-old practice of growing trees and crops and sometimes animals in interacting combinations – that has been ignored in the single-commodity-oriented agricultural and forestry development paradigms – has been brought into the realm of modern land-use. Today agroforestry is well on its way to becoming a specialized science at a level similar to those of crop science and forestry science. To most land-use experts, however, agroforestry has a tropical connotation. They consider agroforestry as something that can and can only be identified with the tropics. That is a wrong perception. While it is true that the tropics, compared to the temperate regions, have a wider array of agroforestry systems and hold greater promise for potential agroforestry interventions, it is also true that agroforestry has several opportunities in the temperate regions too. Indeed, the role of agroforestry is now recognized in Europe as exemplified by this book, North America, and elsewhere in the temperate zone. Current interest in ecosystem management in industrialized countries strongly suggests that there is a need to embrace and apply agroforestry principles to help mitigate the environmental problems caused or exacerbated by commercial agricultural and forestry production enterprises.
