Contexte : Le courrier de sortie des urgences est un des outils de la communication ville-hôpital. Bien que recommandé, il n'est pas toujours réalisé, n'arrive pas toujours à son destinataire ou souvent dans un délai trop long. Son contenu est à l'appréciation de son rédacteur et peut donc être très hétérogène. Objectif : L'objectif principal de cette étude était d'obtenir un consensus sur les critères de réalisation et de qualité des courriers de sortie des urgences de Sallanches. Méthode : Une étude prospective, monocentrique, qualitative, analytique par méthode Delphi® a été menée de janvier à juin 2019, auprès d'un groupe de 42 experts médecins généralistes, infirmiers à domicile et médecins urgentistes travaillant dans la région de Sallanches. Le principe était de proposer des questionnaires consécutifs avec des items à évaluer selon leur pertinence, à l'aide d'une échelle numérique de Likert (1 à 9). Les items obtenant une médiane 8 et moins de 25% de réponses entre 1 et 3 étaient considérés comme validés. Ceux ne faisant pas consensus étaient reformulés ou modifiés, puis proposés dans le questionnaire suivant. Résultats : Le taux de participation des experts a été de 100%. A l'issue de 3 questionnaires, 30 critères ont fait consensus. Un rapport synthétique a été réalisé pour faciliter la rédaction des courriers. Conclusion : Cette étude a permis d'obtenir un consensus sur 30 critères de réalisation et de qualité des courriers de sortie des urgences. La mise en application nécessitera un travail d'uniformisation des pratiques au sein du service des urgences et l'installation de solutions informatiques permettant une réalisation du courrier simple et rapide.

Introduction : L'objectif principal de cette étude était de déterminer les informations susceptibles de prévenir ou de favoriser la survenue des réadmissions dans les trente jours d'une sortie d'hospitalisation. L'objectif secondaire était de décrire les causes de ces réadmissions. METHODE : Etude rétrospective monocentrique au CHU de Grenoble. Les dossiers de réadmission aux urgences en janvier 2013 et les courriers de sortie leur correspondant ont été analysés. La pertinence des admissions était évaluée par l'Appropriateness evaluation protocol (AEP) et l'évitabilité était cotée selon une classification ad hoc des admissions pour une affection déjà connue dérivé de Halfon et al. RESULTATS : Sur les 4169 passages aux urgences, 112 patients étaient sortis d'hospitalisation dans les 30 jours précédents. 30% des admissions étaient “non pertinentes” et 20% étaient évitables. La qualité du courrier de sortie et un antécédent d'hypertension artérielle était statistiquement lié à la survenue d'une admission non pertinente. DISCUSSION : Le taux de réadmission évitable était comparable à la valeur prédite par le modèle proposé par la plus grande méta-analyse à ce jour. L'importance du courrier de sortie a été décrite dans la littérature. La présence d'un cancer, d'une insuffisance cardiaque ou rénale ont été décrit comme facteur de risque indépendant de la survenue d'une réadmission précoce. CONCLUSION : Cette étude souligne l'importance du courrier de sortie dans la communication entre hôpital et soin primaire. Elle met également en avant que malgré une amélioration des courriers de sortie au CHU de Grenoble ces 15 dernières années, des améliorations restent possibles.
