Les consultations aux urgences pédiatriques augmentent chaque année en France. Nous avons réalisé une étude épidémiologique prospective aux urgences pédiatriques du CHU de Caen en 2016, pour connaître les caractéristiques socio-démographiques des usagers, leurs parcours de soins et leurs motifs de consultation. Un questionnaire anonyme était distribué aux accompagnants de chaque enfant qui consultait aux urgences durant les 2 semaines de recueil (une estivale, une hivernale). Nous avons recueilli 704 questionnaires. Nos résultats ont montré qu'une majorité de consultations étaient des consultations simples sans urgence (69,2%), aboutissant sur un retour à domicile (82%). Le taux d'hospitalisation était de 9,6% mais lorsque les patients étaient adressés par un professionnel de santé (35,8%) le taux était de 16,8%, de 14,2% lorsqu'ils avaient déjà consulté antérieurement. Les motifs de consultation étaient multiples : gravité de l'état de santé de l'enfant 32,5%, souhait d'un avis spécialisé 26,4%, indisponibilité du médecin traitant 22,3% même si les symptômes étaient apparus depuis moins de 3 heures (24,2%). Aucun traitement n'était débuté avant de venir aux urgences dans 62,6% des cas, contre 39% si le patient avait déjà consulté antérieurement. Le système de tri initial effectué à l'accueil des enfants semblait efficient, avec une bonne corrélation entre la graduation initiale de gravité et le devenir de l'enfant. Des axes d'amélioration sont envisageables pour diminuer le nombre de consultations aux urgences pédiatriques : création d'un circuit court en poste avancé, campagnes d'information sur le parcours de soins et la place fondamentale de la médecine de ville.
